January                   Welcome to the January edition of Crisis Management’s RecallRegister - Aon’s monthly recall and product
                          safety newsletter. This publication provides a review of the month’s recalls as reported by the U.S. Consumer
2012                      Product Safety Commission (CPSC), the U.S Food and Drug Administration (FDA), the U.S. Department
                          of Agriculture (USDA), National Highway Traffic Safety Administration (NHTSA) and the Canadian Food
                          Inspection Agency (CFIA).

                          In addition to recall announcements, RecallRegister provides an update on the product recall and
                          contamination insurance marketplace and environment. Each month, we highlight an issue of importance
                          including new markets and capacity, significant recall events and changes in legislation affecting the
                          consumer products industry.


                          2011 Ranks Among Worst Years for Food Borne Illness
                          The past year has seen a number of significant illnesses and deaths in the U.S. and Europe as a result of
                          contaminated food products. According to data reported by the U.S. Center for Disease Control (CDC),

                          of hospitalization and death tracked by FoodNet. In 2010, the incidence of Salmonella was nearly three times

                          the 2010 national health objective target. Salmonella can contaminate a wide range of foods. There are many

                          The most significant U.S. food contamination incident in 2011 occurred as a result of listeria tainted cantaloupes.

                          The outbreak, eventually traced back to a Colorado farm, was first detected by the CDC in September. As of
t + 1 312 381 4945        December 8th when the CDC issued its final update on the outbreak, a total of 146 infections from 28 states
e bernie.steves@aon.com   were reported leading to 30 deaths and one miscarriage. This incident has become the most deadly U.S.
                          outbreak of food borne illness since the CDC began tracking listeria cases in 1973. The CDC has advised that

t + 1 312 381 4470        On August 3rd, Cargill Meat Solutions Corporation announced a voluntary recall of approximately 36 million
e kevin.velan@aon.com     pounds of ground turkey because of potential salmonella bacteria contamination. About 5 billion pounds of
                          ready-to-cook turkey meat are sold in the United States each year, of which 10% is ground. The recalled meat
                          represents slightly more than 6% of the national production of ground turkey in a given year. Cargill’s plant in

                          Springdale Arkansas, processed the suspected fresh and frozen ground turkey products between February 20
t + 1 312 381 4849        and August 2, 2011. As of November 10th when the CDC issued its final update on the outbreak, a total of 136
e latia.howard@aon.com    persons infected with the outbreak strain of Salmonella Heidelberg were reported from 34 states including one
                          death. According to the CDC, this particular outbreak appears to be over.

                          Overseas, European farmers found themselves implicated in a devastating E. coli outbreak that first surfaced in
                          Germany in early May. More than 4,000 people were stricken with illness, nearly all of whom lived in Germany
                          or had traveled there. At least 48 people died, according to the European Center for Disease Prevention and
                          Control. European investigators cautiously identified the likely source as contaminated fenugreek seeds from
                          Egypt. Officials also said that the seeds seemed to have entered Europe through a single German importer,
                          which acted as a distributor to other companies.

However, some encouraging data also reported by the CDC showed that as a group, infections caused by six key
pathogens in 2010 were 23% lower, including:

„„   Campylobacter – 27% decrease
„„   Listeria – 38% decrease
„„   E. coli O157 – 44% decrease
„„   Shigella – 57% decrease
„„   Yersinia – 52% decrease

Nevertheless, despite this encouraging data, food borne outbreaks and recalls will continue to be a focus of
the media and press.

December Recall Announcements
The following recalls have been announced through the U.S. Food and Drug Administration (FDA), the U.S.
Department of Agriculture (USDA), the Consumer Product Safety Commission (CPSC), National Highway Traffic
Safety Administration (NHTSA) and the Canadian Food Inspection Agency (CFIA). These agencies are charged
with protecting consumers from products that may pose injury to the public. This is only a representative
list of recalls announced by these agencies during the month. A complete list of recall, as well as actual press
releases, are available at www.recalls.gov and http://www.inspection.gc.ca/english/toce.shtml

California firm recalls Pureed Chicken products due to misbranding and
undeclared allergen
December 2, 2011 - Popkoff Frozen Food, dba Blossom Foods, a San Francisco, Calif. establishment is recalling
approximately 500 pounds of frozen, pureed chicken products because of misbranding and an undeclared
allergen. The products contain the allergen, soy, which is not declared on the label. They were sold to
institutions in the San Francisco and Oakland, Calif. areas and directly to consumers via the Internet. The
problem was discovered by FSIS during a routine label review at the establishment and may have resulted
from a change in chicken base. FSIS and the company have received no reports of adverse reactions due to
consumption of these products.

Flowers Foods pulls Nature’s Own Butterbread off store shelves in certain
Florida and Southeast Georgia Counties
December 2, 2011 - THOMASVILLE, GA - Flowers Foods today announced that, out of an abundance of
caution, it has pulled off store shelves in certain Florida and southeast Georgia counties loaves of its Nature’s
Own Butterbread because they may contain small flakes of aluminum. No other Nature’s Own bakery
products are affected.

No injuries have been reported. Only a small number of loaves may have been affected, and the company has
already pulled loaves with the specified date and code from the counties noted. Grocers in these counties are
advised to withdraw from sale any Nature’s Own Butterbread with the date and code noted that may still be in
their possession and hold it for pickup by the company. The production issue related to this matter has been
corrected.

Ford is recalling certain replacement fuel tanks
Ford is recalling certain replacement fuel tanks that could be installed into model year 2004 through 2008
E-150, E-250, and E-350 vans and wagons. The affected fuel tanks have an extra OBDII sensor hole on the top
of the tank that may not be properly sealed and may leak fuel. A fuel leak in the presence of an ignition source
may result in a fire. Ford will notify owners, and dealers will replace the fuel tanks free of charge. The safety
recall began on November 28, 2011.

Golden Glen Creamery issues voluntary recall of butter because of possible
health risk
December 3, 2011 -Golden Glen Creamery of Bow, WA is voluntarily recalling Butter produced on November 2,
2011 because it has the potential to be contaminated with Listeria monocytogenes.

To date, no illnesses have been reported. The Butters are being voluntarily recalled by Golden Glen Creamery,
as a result of a surveillance sample collected and analyzed by the Washington State Department of Agriculture
that was found to be positive for Listeria monocytogenes. The company has ceased the production and
distribution of the product as FDA, the Washington State Department of Agriculture and the company
continues their investigation as to what caused the problem.

Ocean Spray voluntarily recalls limited quantity of Original Flavor Craisins
Dried Cranberries in Western U.S. only
December 2, 2011 - Ocean Spray today added five additional Best By code dates to its voluntary recall issued
November 25, 2011 of a limited number of production lots of its Original Flavor Craisins® Dried Cranberries
in 5-ounce, 10-ounce and 48-ounce packages in the western U.S. only. Ocean Spray took this precautionary
measure due to the possible presence of very small hair-like metal fragments that are unlikely to cause
consumer injury. The additional five recalled lots were only shipped to retailers in the following eight western
U.S. states: Arizona, California, Colorado, Montana, Nevada, New Mexico, Oregon, and Washington.

Nissan is recalling certain model year 2011 and 2012 Infiniti G37 Coupe
Nissan is recalling certain model year 2011 and 2012 Infiniti G37 Coupe vehicles and certain model year 2011
Nissan 370z vehicles manufactured from July 2, 2011, through August 29, 2011. Due to a programming error, the
power window switch controller in the subject vehicles was manufactured out of specification. The threshold for
auto reverse activation is higher than the Nissan specification. Should a body part become trapped by a closing
window, a pinch injury may result. Nissan will notify owners and dealers and will replace the power window
switches with new ones free of charge. The safety recall began on December 5, 2011

Mars Food US recalls two date codes of Uncle Ben’s Whole Grain White Rice
Garden Vegetable due to undeclared milk
December 6, 2011 - Mars Food US today issued a voluntary recall of two date codes of UNCLE BEN’s® Whole
Grain White Rice Garden Vegetable due to an undeclared milk allergen. People who have an allergy or severe
sensitivity to milk run the risk of serious or life-threatening allergic reaction if they consume these products. No
other flavors of UNCLE BEN’s® Whole Grain White Rice or any other UNCLE BEN’s® products are affected.

This product was distributed through retail stores in Alabama, Arkansas, Arizona, California, Colorado, Delaware,
Florida, Georgia, Iowa, Illinois, Indiana, Kansas, Kentucky, Louisiana, Maine, Michigan, Minnesota, Missouri,
Mississippi, North Carolina, New Hampshire, New York, Ohio, Oregon, Pennsylvania, Tennessee, Texas, Utah,
Virginia, Vermont and Wisconsin. An ingredient supplier improperly included an undeclared milk ingredient in
the seasoning mix used for this product. Consumers with milk allergies are advised not to eat the product. All
consumers should return the product to the store where it was purchased for a full refund.

GM is recalling certain model year 2012 Cadillac CTS vehicles
GM is recalling certain model year 2012 Cadillac CTS vehicles manufactured from October 21, 2011, through
October 26, 2011. These vehicles may have a condition in which the power vacuum brake boost pushrod
retention nut may not be torqued to the proper specification. If the nut is not torqued to the proper
specification, the nut could loosen and allow the pushrod to separate from the brake pedal. This could result
in a loss of ability to brake and a vehicle crash without prior warning. Cadillac dealers will inspect the power
vacuum brake boost pushrod retention nut, and if necessary, torque the nut to the proper specification. This
service will be performed free of charge.

See’s Candies Inc issues allergy alert on 8 ounce Almond Clusters, Code SF
088/12
December 6, 2011 - See’s Candies, Inc. of San Francisco, Calif., is recalling one code of 8 ounce Almond
Clusters, because they may contain peanuts. People who have an allergy or severe sensitivity to peanuts run
the risk of serious or life-threatening allergic reaction if they consume this product. The total number of boxes
produced that may contain peanuts is 3,600.

P&G voluntarily recalls one production lot of Dry Dog Food
December 6, 2011 - The Procter & Gamble Company (P&G) has voluntarily retrieved a single production
lot of dry dog food due to aflatoxin levels that were detected above the acceptable limit. This product has
already been retrieved from store shelves. No illnesses have been reported in association with this production
lot to date, and no other Iams pet food products are involved.These retailers have already removed this
product from store shelves. No other dry dog food, dry cat food, dog or cat canned food, biscuits/treats or
supplements are affected by this announcement.

New Jersey firm recalls Frozen Stuffed Pasta Products produced without
benefit of inspection
Dec 8, 2011 - D’Orazio Foods, Inc., a Bellmawr, N.J., establishment, is recalling approximately 161,000 pounds
of frozen stuffed pasta products that were produced without the benefit of federal inspection, the U.S.
Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The problem was
discovered as part of a routine food safety assessment conducted by FSIS at the facility. While conducting the
assessment, the Agency determined that the pasta products were produced during a second shift when FSIS
inspection personnel were not on-site, as required, during operating hours.

North Carolina firm recalls Oven Roasted Chicken Breast for possible
Listeria Contamination
Dec. 9, 2011 - House of Raeford Farms, a Raeford, N.C. establishment, is recalling approximately 4,140 pounds
of cooked chicken breasts that may be contaminated with Listeria monocytogenes, the U.S. Department of
Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The problem was discovered after a
customer’s laboratory sample tested positive for Listeria monocytogenes. FSIS has received no reports of illness due
to consumption of these products. Anyone concerned about an illness should contact a health care provider.

Voluntarily recall of limited quantity of 12 oz Tejava® Premium Iced Tea
December 9, 2011 – As a precautionary measure, certain production lots of Tejava® Premium Iced Tea in 12
oz glass bottles are being recalled nationwide due to the possible presence of glass fragments which could
be harmful if swallowed. To date, there have been no reports of any illness or injury. Consumers who have
purchased the recalled products are asked to return the product to the store where it was purchased and
coupons, product replacement, or credit will be issued.

Certain sliced Mushrooms may contain Listeria Monocytogenes
December 10, 2011 - The Canadian Food Inspection Agency (CFIA) and Greenwood Mushroom Farm are
warning the public not to consume the sliced mushrooms described below because the product may be
contaminated with Listeria monocytogenes. These products have been distributed in Ontario. There have
been no reported illnesses associated with the consumption of these products. The processor, Greenwood
Mushroom Farm, Ashburn, ON is voluntarily recalling the affected products from the marketplace. The CFIA is
monitoring the effectiveness of the recall.

Certain Bos brand Refrigerated Vacuum Packed Smoked Fish products
without best before dates may contain dangerous bacteria
December 12, 2011 - The Canadian Food Inspection Agency (CFIA) and Bos Smoked Fish Inc. are warning the
public not to consume certain Bos brand refrigerated vacuum packed smoked fish products without Best
Before dates, described below, because the products may be contaminated with Clostridium botulinum.
Toxins produced by this bacteria may cause botulism, a life-threatening illness.

All Bos brand smoked vacuum packed fish products, sold refrigerated in various weight packages without
a Best Before Date, are affected by the alert. This includes all species and flavours of the Bos brand smoked
vacuum packed fish products with “Keep Refrigerated” on the package. These products have been distributed
in Ontario. There have been no reported illnesses associated with the consumption of these products.

E-Z-GO recalls Golf cars, Shuttles and Off-Road utility vehicles due to crash
hazard
December 14, 2011- The U.S. Consumer Product Safety Commission, in cooperation with E-Z-GO, a Textron
Company, of Augusta, Ga, today announced a voluntary recall of TXT golf cars, Cushman shuttle vehicles
and Bad Boy off-road utility vehicles. Consumers should stop using recalled products immediately unless
otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

The threaded end of the rack rod ball joint can break and the ball joint can become displaced, causing the
driver to lose steering control. This can result in a crash. The firm is aware of 71 reports of the ball joint
breaking, 13 of which resulted in the ball joint displacing. No injuries have been reported. The recalled
vehicles are gas- and electric-powered, four-wheeled vehicles with bench seats for the driver and passengers.
Consumers should immediately stop using the recalled vehicles and contact E-Z-GO or an authorized dealer
for a free repair. E-Z-GO and E-Z-GO dealers are contacting known owners.

Starwest Botanicals recalls Starwest Organic Celery Seed due to potential
Salmonella Contamination
December 14, 2011 - Starwest Botanicals is voluntarily recalling Starwest Organic Celery Seed (Whole)
because it has potential to be contaminated with Salmonella. The product was sold by several different
methods. It was either shipped directly to on-line or mail order consumers, or held for pickup by consumers
at Starwest Botanicals. The product was sold between April 26, 2011 and December 14, 2011. Starwest
Botanicals became aware of the potential contamination after being notified by a customer. No other
Starwest spices have been affected by this voluntary recall. No illnesses have been reported to date in
connection with this voluntary recall.

Winn-Dixie issues voluntary recall on Bulk Gummy Bears
December 14, 2011 - Winn-Dixie Stores, Inc. (NASDAQ: WINN), is voluntarily recalling Sunrise Assorted Flavor
Gummy Bears sold in the self-serve bulk area of select store produce departments between November 14
and December 13 due to a remote possibility of metal contamination. The recall is being initiated out of an
abundance of caution for consumer safety, though the company has received no reports of illness associated
with product consumption.

North Carolina firm recalls Meatloaf Entrées due to misbranding and an
undeclared allergen
Dec. 15, 2011 - B. Roberts Foods, a Charlotte, N.C. establishment, is recalling approximately 130 pounds of
individual serving meatloaf entrées because of misbranding and an undeclared allergen, the U.S. Department
of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The product contains anchovies,
a known allergen, which is not noted on the label. A grocery store employee discovered that the top and
bottom labels on the entrees did not match. The grocery store reported the error to B. Roberts Foods, which
found that the “Meatloaf with Red Skin Mashed Potatoes” entrées had been given bottom labels intended
for “Homestyle Meatloaf with Tomato Sauce” due to an employee error. The recalled products are made with
Worcestershire sauce that includes anchovies, which are allergenic and must be noted on the label.

Northeastern Grocery Chain recalls Ground Beef products due to possible
Salmonella Contamination
December. 15, 2011 - Hannaford, a Scarborough, Maine-based grocery chain, is recalling an undetermined
amount of fresh ground beef products that may be contaminated with a strain of Salmonella Typhimurium,
the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.

This recall was initiated due to concerns about illnesses caused by an outbreak of salmonellosis that may be
associated with use and consumption of fresh in-store ground beef prepared in and purchased at Hannaford
stores. The PFGE pattern associated with this outbreak is reported rarely in the U.S. The Centers for Disease
Control and Prevention (CDC) have reported 14 ill persons with an indistinguishable PFGE pattern, and 11
of those individuals reported consuming ground beef. Seven individuals were hospitalized and no deaths
have been reported. Ten of the 14 case-patients reported purchasing ground beef at Hannaford stores in
Maine, New York, New Hampshire and Vermont between Oct. 12 and Nov. 20. No other product description is
available at this time. As a result of on-going epidemiologic and traceback investigations, as well as in-store
reviews, FSIS, working with the CDC and its state health partners, determined that there is a link between
the Hannaford ground beef products and this illness outbreak. FSIS is continuing to work with CDC, public
health partners in the affected states and the company on the investigation. FSIS will continue to provide
information as it becomes available.

Toasters recalled by Hamilton Beach due to fire hazard
December 15, 2011 -The U.S. Consumer Product Safety Commission, in cooperation with Hamilton Beach
Brands Inc., of Glen Allen, Va., today announced a voluntary recall of Hamilton Beach® classic chrome 2-slice
toasters. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal
to resell or attempt to resell a recalled consumer product.

When the toasters are first plugged into the outlets, the heating element can be energized although the toaster
lifter is in the up or off position, which can pose a fire hazard if the toaster is near flammable items. Hamilton
Beach has received 5 reports of toasters being energized when first plugged into an outlet. There have been no
reports of injuries or property damage. The Hamilton Beach recall involves model 22602 toasters.

Northeastern Grocery chain recalls Ground Beef products due to possible
Salmonella Contamination

Nebraska Firm recalls Ground Beef products due to possible E. Coli
O157:H7 Contamination
December 16, 2011 - Tyson Fresh Meats Inc., a Dakota City, Neb. establishment, is recalling approximately
40,948 pounds of ground beef products that may be contaminated with E. coli O157:H7, the U.S. Department
of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The problem was discovered
through routine FSIS monitoring which confirmed a positive result for E. coli O157:H7. FSIS and the company
have received no reports of illnesses associated with consumption of these products.

Swanson Health products recalls Swanson Organic Celery Seed
December 16, 2011 - Swanson Health Products is voluntarily recalling Swanson Organic Celery Seed (Whole)
which is packaged in plastic bottles with a net weight of 1.4 oz. (40 grams) because it has the potential to be
contaminated with Salmonella.

The product was sold by several different methods. It was either sold at the Swanson Health Products retail
store, 109 Broadway N., Fargo, ND, shipped directly to on-line or mail-order consumers, or held for pick up by
the consumer at Swanson Health Products headquarters. The product was sold between June 16, 2011, and
December 16, 2011.

Swanson Health Products became aware of the potential contamination after being notified by its supplier, B &
M, Inc., Mount Vernon, Missouri. B & M, Inc. had earlier learned of the potential contamination from its supplier.

The Pampered Chef recalls Ice Cream Dippers due to impact injury hazard
December 20, 2011- The U.S. Consumer Product Safety Commission, in cooperation with The Zeroll Company, of
Fort Pierce, Fla, today announced a voluntary recall of Ice cream dippers. Consumers should stop using recalled
products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer
product. When the liquid-filled ice cream scoop is exposed to warm water, the cap and seal at the end of the
scoop handle can fly off with substantial force, posing an impact injury hazard to nearby consumers.

The Pampered Chef has received 16 reports including damage to kitchen items and six reports of personal
injuries including lacerations, bruises and redness caused by caps coming off the base of the handle. The
dippers are metallic gray aluminum with a plastic dome-shaped cap on the base of the handle. “Pampered
Chef” is imprinted in the cap. The ice cream dippers measure around seven inches in length. A non-toxic liquid
is sealed inside the handle of the product which conducts the heat of your hand to more easily scoop hard ice
cream. Consumers should immediately stop using the recalled ice cream dippers and contact The Pampered
Chef for instructions on obtaining a replacement or refund. Consumers can also contact their Pampered Chef
consultant for replacement and refund information.

Cal Fresco LLC recalls Jalapeño and Serrano Chili Peppers due to possible
health risk
December 21, 2011 - Cal Fresco, LLC (“Cal Fresco”) is voluntarily working with the Food and Drug
Administration (“FDA”) and California Department of Public Health (“CDPH”) to coordinate a recall of fresh
Jalapeño and Serrano chili peppers for potential contamination with Salmonella. The FDA has advised that a
random sample of Cal Fresco Jalapeño and Serrano peppers has tested positive for Salmonella.

The product is typically sold at the retail level in bulk by weight and has no retail packaging associated with it.
All retail suppliers that received this affected lot have been notified and were directed to immediately remove
and destroy any remaining product in their inventories. This recall was the result of a random sampling event
on December 12, 2011 by the USDA which revealed the presence of Salmonella on some of the product. Cal
Fresco’s recall and traceability program enabled the company to quickly identify the supplier of the affected
product, which originated in Mexico. Cal Fresco has ceased the distribution of all produce from this supplier
while the FDA, the California Department of Public Health, and the company continue their investigation
into the source of the contamination. No illnesses have been reported to date. Other than the Jalapeño and
Serrano peppers described above, no other Cal Fresco product has been affected by this recall.

McNeil recall of Motrin® IB Tablets and Caplets
December 21, 2011 - McNeil Consumer Healthcare Division of McNEIL-PPC, Inc. (McNeil) is voluntarily recalling
certain lots of MOTRIN® IB 24 count COATED CAPLETS, MOTRIN® IB 24 count COATED TABLETS and MOTRIN® IB
24+6 count COATED CAPLETS from retailers. This is not a consumer level recall, which means that consumers
do not need to dispose of or return the product. There is no safety concern if consumers continue taking the
product in accordance with its label; however, it is possible there may be a delay in experiencing relief. This
action is not being undertaken on the basis of adverse events. McNeil is recalling these products because
testing of product samples showed that some caplets may not dissolve as quickly as intended when nearing
their expiration date.

Holiday Tree, Wreath and Garland Set recalled by General Foam Plastic
Corp. due to fire hazard sold exclusively at True Value Hardware Stores
December 24, 2011 - The U.S. Consumer Product Safety Commission, in cooperation with General Foam Plastic
Corp., of Norfolk, Va, today announced a voluntary recall of the Brookfield Entry Way Tree Sets. Consumers
should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt

The battery box that power lights on the artificial wreath can overheat, posing a fire hazard. The firm has
received 12 reports of battery boxes overheating and being warm to the touch after batteries were installed.
No injuries have been reported. Consumers should immediately stop using the recalled products and return
them to True Value for a full refund.

West Elm recalls Dover Dining Table due to injury hazard
December 28, 2011 - The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with
West Elm, a division of Williams-Sonoma, Inc., of San Francisco, today announced a voluntary recall of the
Dover Dining Table. Consumers should stop using recalled products immediately unless otherwise instructed.
It is illegal to resell or attempt to resell a recalled consumer product.

The wooden base of the table can collapse, causing the glass table top to fall. This poses an injury hazard to
consumers. The firm is aware of 14 reports of tables collapsing or breaking, including one report of a bruise to
a consumer’s leg. The Dover dining table is a tempered glass-top table with a brown wooden base. The base
has four legs, four buttress support bars and three glasstop support bars. “West Elm” and “SKU: 2188233” are
printed on a white label on the underside of the long, center beam. Consumers should contact West Elm for a
full store credit. West Elm will schedule a free return pickup of the table.

Green Valley Food Corp. is recalling approximately 35,159 cases of a variety of products because they have
the potential to be contaminated with Listeria monocytogenes. Random samples tested positive for Listeria
monocytogenes

On 12/12/2011 a random sample was taken from a customer the product tested positive for Salmonella.
Green Valley Food Corp. voluntarily recalled the alfalfa-based products when we were notified of the positive
result on 12/23/2011 and 12/24/2011. On 12/21/2011 random samples were taken of our facility, several
samples had a positive result for Listeria monocytogenes. To assure our customers of safe products, Green
Valley Food Corp., is voluntarily recalling the additional items listed, which consist of “Let’s Grow Healthy
Together”, “Green Valley Food Corp.” and “BroccoSprouts” branded products. This is a voluntary cautionary
measure taken by Green Valley Food Corp. to assure all customers and consumers of any issues with our
products. We are voluntarily requesting this recall and are working closely with the FDA to assure all
consumers our products are safe.

  • 1. RecallRegister January Welcome to the January edition of Crisis Management’s RecallRegister - Aon’s monthly recall and product safety newsletter. This publication provides a review of the month’s recalls as reported by the U.S. Consumer 2012 Product Safety Commission (CPSC), the U.S Food and Drug Administration (FDA), the U.S. Department of Agriculture (USDA), National Highway Traffic Safety Administration (NHTSA) and the Canadian Food Inspection Agency (CFIA). In addition to recall announcements, RecallRegister provides an update on the product recall and contamination insurance marketplace and environment. Each month, we highlight an issue of importance including new markets and capacity, significant recall events and changes in legislation affecting the consumer products industry. 2011 Ranks Among Worst Years for Food Borne Illness The past year has seen a number of significant illnesses and deaths in the U.S. and Europe as a result of contaminated food products. According to data reported by the U.S. Center for Disease Control (CDC), Aon Contacts Salmonella was the most common infection (1.2 million U.S. illnesses annually) and the most common cause of hospitalization and death tracked by FoodNet. In 2010, the incidence of Salmonella was nearly three times Bernie Steves the 2010 national health objective target. Salmonella can contaminate a wide range of foods. About 5 billion pounds of ready-to-cook turkey meat are sold in the United States each year, of which 10% is ground. The recalled meat represents slightly more than 6% of the national production of ground turkey in a given year. Cargill’s plant in Latia Howard Springdale Arkansas, processed the suspected fresh and frozen ground turkey products between February 20 t + 1 312 381 4849 and August 2, 2011. As of November 10th when the CDC issued its final update on the outbreak, a total of 136 e latia.howard@aon.com persons infected with the outbreak strain of Salmonella Heidelberg were reported from 34 states including one death. According to the CDC, this particular outbreak appears to be over. Overseas, European farmers found themselves implicated in a devastating E. coli outbreak that first surfaced in Germany in early May. More than 4,000 people were stricken with illness, nearly all of whom lived in Germany or had traveled there. At least 48 people died, according to the European Center for Disease Prevention and Control. European investigators cautiously identified the likely source as contaminated fenugreek seeds from Egypt. Officials also said that the seeds seemed to have entered Europe through a single German importer, which acted as a distributor to other companies. RecallRegister | January 2012

However, some encouraging data also reported by the CDC showed that as a group, infections caused by six key pathogens in 2010 were 23% lower, including: „„ Campylobacter – 27% decrease „„ Listeria – 38% decrease „„ E. coli O157 – 44% decrease „„ Shigella – 57% decrease „„ Yersinia – 52% decrease Nevertheless, despite this encouraging data, food borne outbreaks and recalls will continue to be a focus of the media and press. Source: U.S. Center for Disease Control (www.cdc.gov), U.S. Food & Drug Administration (www.fda.gov) December Recall Announcements The following recalls have been announced through the U.S. Food and Drug Administration (FDA), the U.S. Department of Agriculture (USDA), the Consumer Product Safety Commission (CPSC), National Highway Traffic Safety Administration (NHTSA) and the Canadian Food Inspection Agency (CFIA). These agencies are charged with protecting consumers from products that may pose injury to the public. This is only a representative list of recalls announced by these agencies during the month. A complete list of recall, as well as actual press releases, are available at www.recalls.gov and http://www.inspection.gc.ca/english/toce.shtml California firm recalls Pureed Chicken products due to misbranding and undeclared allergen December 2, 2011 - Popkoff Frozen Food, dba Blossom Foods, a San Francisco, Calif. establishment is recalling approximately 500 pounds of frozen, pureed chicken products because of misbranding and an undeclared allergen. The products contain the allergen, soy, which is not declared on the label. They were sold to institutions in the San Francisco and Oakland, Calif. areas and directly to consumers via the Internet. The problem was discovered by FSIS during a routine label review at the establishment and may have resulted from a change in chicken base. FSIS and the company have received no reports of adverse reactions due to consumption of these products. Flowers Foods pulls Nature’s Own Butterbread off store shelves in certain Florida and Southeast Georgia Counties December 2, 2011 - THOMASVILLE, GA - Flowers Foods today announced that, out of an abundance of caution, it has pulled off store shelves in certain Florida and southeast Georgia counties loaves of its Nature’s Own Butterbread because they may contain small flakes of aluminum. No other Nature’s Own bakery products are affected. No injuries have been reported. Only a small number of loaves may have been affected, and the company has already pulled loaves with the specified date and code from the counties noted. Grocers in these counties are advised to withdraw from sale any Nature’s Own Butterbread with the date and code noted that may still be in their possession and hold it for pickup by the company. Ford is recalling certain replacement fuel tanks Ford is recalling certain replacement fuel tanks that could be installed into model year 2004 through 2008 E-150, E-250, and E-350 vans and wagons. The affected fuel tanks have an extra OBDII sensor hole on the top of the tank that may not be properly sealed and may leak fuel. A fuel leak in the presence of an ignition source may result in a fire. Ford will notify owners, and dealers will replace the fuel tanks free of charge. The safety recall began on November 28, 2011. Golden Glen Creamery issues voluntary recall of butter because of possible health risk December 3, 2011 -Golden Glen Creamery of Bow, WA is voluntarily recalling Butter produced on November 2, 2011 because it has the potential to be contaminated with Listeria monocytogenes. To date, no illnesses have been reported. The Butters are being voluntarily recalled by Golden Glen Creamery, as a result of a surveillance sample collected and analyzed by the Washington State Department of Agriculture that was found to be positive for Listeria monocytogenes. The company has ceased the production and distribution of the product as FDA, the Washington State Department of Agriculture and the company continues their investigation as to what caused the problem. Ocean Spray voluntarily recalls limited quantity of Original Flavor Craisins Dried Cranberries in Western U.S. only December 2, 2011 - Ocean Spray today added five additional Best By code dates to its voluntary recall issued November 25, 2011 of a limited number of production lots of its Original Flavor Craisins® Dried Cranberries in 5-ounce, 10-ounce and 48-ounce packages in the western U.S. only. Ocean Spray took this precautionary measure due to the possible presence of very small hair-like metal fragments that are unlikely to cause consumer injury. The additional five recalled lots were only shipped to retailers in the following eight western U.S. states: Arizona, California, Colorado, Montana, Nevada, New Mexico, Oregon, and Washington. Nissan is recalling certain model year 2011 and 2012 Infiniti G37 Coupe Nissan is recalling certain model year 2011 and 2012 Infiniti G37 Coupe vehicles and certain model year 2011 Nissan 370z vehicles manufactured from July 2, 2011, through August 29, 2011. Due to a programming error, the power window switch controller in the subject vehicles was manufactured out of specification. The threshold for auto reverse activation is higher than the Nissan specification. Should a body part become trapped by a closing window, a pinch injury may result. Nissan will notify owners and dealers and will replace the power window switches with new ones free of charge. The safety recall began on December 5, 2011 Mars Food US recalls two date codes of Uncle Ben’s Whole Grain White Rice Garden Vegetable due to undeclared milk December 6, 2011 - Mars Food US today issued a voluntary recall of two date codes of UNCLE BEN’s® Whole Grain White Rice Garden Vegetable due to an undeclared milk allergen. People who have an allergy or severe sensitivity to milk run the risk of serious or life-threatening allergic reaction if they consume these products. No other flavors of UNCLE BEN’s® Whole Grain White Rice or any other UNCLE BEN’s® products are affected. This product was distributed through retail stores in Alabama, Arkansas, Arizona, California, Colorado, Delaware, Florida, Georgia, Iowa, Illinois, Indiana, Kansas, Kentucky, Louisiana, Maine, Michigan, Minnesota, Missouri, Mississippi, North Carolina, New Hampshire, New York, Ohio, Oregon, Pennsylvania, Tennessee, Texas, Utah, Virginia, Vermont and Wisconsin. An ingredient supplier improperly included an undeclared milk ingredient in the seasoning mix used for this product. Consumers with milk allergies are advised not to eat the product.   • 4. GM is recalling certain model year 2012 Cadillac CTS vehicles GM is recalling certain model year 2012 Cadillac CTS vehicles manufactured from October 21, 2011, through October 26, 2011. These vehicles may have a condition in which the power vacuum brake boost pushrod retention nut may not be torqued to the proper specification. If the nut is not torqued to the proper specification, the nut could loosen and allow the pushrod to separate from the brake pedal. This could result in a loss of ability to brake and a vehicle crash without prior warning. Cadillac dealers will inspect the power vacuum brake boost pushrod retention nut, and if necessary, torque the nut to the proper specification. This service will be performed free of charge. See’s Candies Inc issues allergy alert on 8 ounce Almond Clusters, Code SF 088/12 December 6, 2011 - See’s Candies, Inc. of San Francisco, Calif., is recalling one code of 8 ounce Almond Clusters, because they may contain peanuts. People who have an allergy or severe sensitivity to peanuts run the risk of serious or life-threatening allergic reaction if they consume this product. The total number of boxes produced that may contain peanuts is 3,600. P&G voluntarily recalls one production lot of Dry Dog Food December 6, 2011 - The Procter & Gamble Company (P&G) has voluntarily retrieved a single production lot of dry dog food due to aflatoxin levels that were detected above the acceptable limit. This product has already been retrieved from store shelves. No illnesses have been reported in association with this production lot to date, and no other Iams pet food products are involved.These retailers have already removed this product from store shelves. No other dry dog food, dry cat food, dog or cat canned food, biscuits/treats or supplements are affected by this announcement. New Jersey firm recalls Frozen Stuffed Pasta Products produced without benefit of inspection Dec 8, 2011 - D’Orazio Foods, Inc., a Bellmawr, N.J., establishment, is recalling approximately 161,000 pounds of frozen stuffed pasta products that were produced without the benefit of federal inspection, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The problem was discovered as part of a routine food safety assessment conducted by FSIS at the facility. While conducting the assessment, the Agency determined that the pasta products were produced during a second shift when FSIS inspection personnel were not on-site, as required, during operating hours. North Carolina firm recalls Oven Roasted Chicken Breast for possible Listeria Contamination Dec. 9, 2011 - House of Raeford Farms, a Raeford, N.C. establishment, is recalling approximately 4,140 pounds of cooked chicken breasts that may be contaminated with Listeria monocytogenes, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The problem was discovered after a customer’s laboratory sample tested positive for Listeria monocytogenes. FSIS has received no reports of illness due to consumption of these products. Anyone concerned about an illness should contact a health care provider. Voluntarily recall of limited quantity of 12 oz Tejava® Premium Iced Tea December 9, 2011 – As a precautionary measure, certain production lots of Tejava® Premium Iced Tea in 12 oz glass bottles are being recalled nationwide due to the possible presence of glass fragments which could be harmful if swallowed. To date, there have been no reports of any illness or injury. Consumers who have purchased the recalled products are asked to return the product to the store where it was purchased and coupons, product replacement, or credit will be issued. Certain sliced Mushrooms may contain Listeria Monocytogenes December 10, 2011 - The Canadian Food Inspection Agency (CFIA) and Greenwood Mushroom Farm are warning the public not to consume the sliced mushrooms described below because the product may be contaminated with Listeria monocytogenes. These products have been distributed in Ontario. There have been no reported illnesses associated with the consumption of these products. The processor, Greenwood Mushroom Farm, Ashburn, ON is voluntarily recalling the affected products from the marketplace. The CFIA is monitoring the effectiveness of the recall. Certain Bos brand Refrigerated Vacuum Packed Smoked Fish products without best before dates may contain dangerous bacteria December 12, 2011 - The Canadian Food Inspection Agency (CFIA) and Bos Smoked Fish Inc. are warning the public not to consume certain Bos brand refrigerated vacuum packed smoked fish products without Best Before dates, described below, because the products may be contaminated with Clostridium botulinum. Toxins produced by this bacteria may cause botulism, a life-threatening illness. All Bos brand smoked vacuum packed fish products, sold refrigerated in various weight packages without a Best Before Date, are affected by the alert. This includes all species and flavours of the Bos brand smoked vacuum packed fish products with “Keep Refrigerated” on the package. These products have been distributed in Ontario. There have been no reported illnesses associated with the consumption of these products. E-Z-GO recalls Golf cars, Shuttles and Off-Road utility vehicles due to crash hazard December 14, 2011- The U.S. Consumer Product Safety Commission, in cooperation with E-Z-GO, a Textron Company, of Augusta, Ga, today announced a voluntary recall of TXT golf cars, Cushman shuttle vehicles and Bad Boy off-road utility vehicles. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product. The threaded end of the rack rod ball joint can break and the ball joint can become displaced, causing the driver to lose steering control. This can result in a crash. The firm is aware of 71 reports of the ball joint breaking, 13 of which resulted in the ball joint displacing. No injuries have been reported. The recalled vehicles are gas- and electric-powered, four-wheeled vehicles with bench seats for the driver and passengers. Consumers should immediately stop using the recalled vehicles and contact E-Z-GO or an authorized dealer for a free repair. E-Z-GO and E-Z-GO dealers are contacting known owners. Starwest Botanicals recalls Starwest Organic Celery Seed due to potential Salmonella Contamination December 14, 2011 - Starwest Botanicals is voluntarily recalling Starwest Organic Celery Seed (Whole) because it has potential to be contaminated with Salmonella. The product was sold by several different methods. It was either shipped directly to on-line or mail order consumers, or held for pickup by consumers at Starwest Botanicals. The product was sold between April 26, 2011 and December 14, 2011. Starwest Botanicals became aware of the potential contamination after being notified by a customer. (NASDAQ: WINN), is voluntarily recalling Sunrise Assorted Flavor Gummy Bears sold in the self-serve bulk area of select store produce departments between November 14 and December 13 due to a remote possibility of metal contamination. The recall is being initiated out of an abundance of caution for consumer safety, though the company has received no reports of illness associated with product consumption. North Carolina firm recalls Meatloaf Entrées due to misbranding and an undeclared allergen Dec. 15, 2011 - B. Roberts Foods, a Charlotte, N.C. establishment, is recalling approximately 130 pounds of individual serving meatloaf entrées because of misbranding and an undeclared allergen, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The product contains anchovies, a known allergen, which is not noted on the label. A grocery store employee discovered that the top and bottom labels on the entrees did not match. The grocery store reported the error to B. Roberts Foods, which found that the “Meatloaf with Red Skin Mashed Potatoes” entrées had been given bottom labels intended for “Homestyle Meatloaf with Tomato Sauce” due to an employee error. The recalled products are made with Worcestershire sauce that includes anchovies, which are allergenic and must be noted on the label. Northeastern Grocery Chain recalls Ground Beef products due to possible Salmonella Contamination December. 15, 2011 - Hannaford, a Scarborough, Maine-based grocery chain, is recalling an undetermined amount of fresh ground beef products that may be contaminated with a strain of Salmonella Typhimurium, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. This recall was initiated due to concerns about illnesses caused by an outbreak of salmonellosis that may be associated with use and consumption of fresh in-store ground beef prepared in and purchased at Hannaford stores. The PFGE pattern associated with this outbreak is reported rarely in the U.S. The Centers for Disease Control and Prevention (CDC) have reported 14 ill persons with an indistinguishable PFGE pattern, and 11 of those individuals reported consuming ground beef. Seven individuals were hospitalized and no deaths have been reported. Ten of the 14 case-patients reported purchasing ground beef at Hannaford stores in Maine, New York, New Hampshire and Vermont between Oct. 12 and Nov. 20. No other product description is available at this time. As a result of on-going epidemiologic and traceback investigations, as well as in-store reviews, FSIS, working with the CDC and its state health partners, determined that there is a link between the Hannaford ground beef products and this illness outbreak. FSIS is continuing to work with CDC, public health partners in the affected states and the company on the investigation. FSIS will continue to provide information as it becomes available. Toasters recalled by Hamilton Beach due to fire hazard December 15, 2011 -The U.S. Consumer Product Safety Commission, in cooperation with Hamilton Beach Brands Inc., of Glen Allen, Va., today announced a voluntary recall of Hamilton Beach® classic chrome 2-slice toasters. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product. When the toasters are first plugged into the outlets, the heating element can be energized although the toaster lifter is in the up or off position, which can pose a fire hazard if the toaster is near flammable items. Hamilton Beach has received 5 reports of toasters being energized when first plugged into an outlet. There have been no reports of injuries or property damage. Coli O157:H7 Contamination December 16, 2011 - Tyson Fresh Meats Inc., a Dakota City, Neb. establishment, is recalling approximately 40,948 pounds of ground beef products that may be contaminated with E. coli O157:H7, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The problem was discovered through routine FSIS monitoring which confirmed a positive result for E. coli O157:H7. FSIS and the company have received no reports of illnesses associated with consumption of these products. Swanson Health products recalls Swanson Organic Celery Seed December 16, 2011 - Swanson Health Products is voluntarily recalling Swanson Organic Celery Seed (Whole) which is packaged in plastic bottles with a net weight of 1.4 oz. (40 grams) because it has the potential to be contaminated with Salmonella. The product was sold by several different methods. It was either sold at the Swanson Health Products retail store, 109 Broadway N., Fargo, ND, shipped directly to on-line or mail-order consumers, or held for pick up by the consumer at Swanson Health Products headquarters. The product was sold between June 16, 2011, and December 16, 2011. Swanson Health Products became aware of the potential contamination after being notified by its supplier, B & M, Inc., Mount Vernon, Missouri. B & M, Inc. had earlier learned of the potential contamination from its supplier. The Pampered Chef recalls Ice Cream Dippers due to impact injury hazard December 20, 2011- The U.S. Consumer Product Safety Commission, in cooperation with The Zeroll Company, of Fort Pierce, Fla, today announced a voluntary recall of Ice cream dippers. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product. When the liquid-filled ice cream scoop is exposed to warm water, the cap and seal at the end of the scoop handle can fly off with substantial force, posing an impact injury hazard to nearby consumers. The Pampered Chef has received 16 reports including damage to kitchen items and six reports of personal injuries including lacerations, bruises and redness caused by caps coming off the base of the handle. The dippers are metallic gray aluminum with a plastic dome-shaped cap on the base of the handle. “Pampered Chef” is imprinted in the cap. The ice cream dippers measure around seven inches in length. A non-toxic liquid is sealed inside the handle of the product which conducts the heat of your hand to more easily scoop hard ice cream. Consumers should immediately stop using the recalled ice cream dippers and contact The Pampered Chef for instructions on obtaining a replacement or refund. Cal Fresco LLC recalls Jalapeño and Serrano Chili Peppers due to possible health risk December 21, 2011 - Cal Fresco, LLC (“Cal Fresco”) is voluntarily working with the Food and Drug Administration (“FDA”) and California Department of Public Health (“CDPH”) to coordinate a recall of fresh Jalapeño and Serrano chili peppers for potential contamination with Salmonella. The FDA has advised that a random sample of Cal Fresco Jalapeño and Serrano peppers has tested positive for Salmonella. The product is typically sold at the retail level in bulk by weight and has no retail packaging associated with it. All retail suppliers that received this affected lot have been notified and were directed to immediately remove and destroy any remaining product in their inventories. This recall was the result of a random sampling event on December 12, 2011 by the USDA which revealed the presence of Salmonella on some of the product. Cal Fresco’s recall and traceability program enabled the company to quickly identify the supplier of the affected product, which originated in Mexico. Cal Fresco has ceased the distribution of all produce from this supplier while the FDA, the California Department of Public Health, and the company continue their investigation into the source of the contamination. No illnesses have been reported to date. Other than the Jalapeño and Serrano peppers described above, no other Cal Fresco product has been affected by this recall. McNeil recall of Motrin® IB Tablets and Caplets December 21, 2011 - McNeil Consumer Healthcare Division of McNEIL-PPC, Inc. (McNeil) is voluntarily recalling certain lots of MOTRIN® IB 24 count COATED CAPLETS, MOTRIN® IB 24 count COATED TABLETS and MOTRIN® IB 24+6 count COATED CAPLETS from retailers. This is not a consumer level recall, which means that consumers do not need to dispose of or return the product. There is no safety concern if consumers continue taking the product in accordance with its label; however, it is possible there may be a delay in experiencing relief. This action is not being undertaken on the basis of adverse events. McNeil is recalling these products because testing of product samples showed that some caplets may not dissolve as quickly as intended when nearing their expiration date. Holiday Tree, Wreath and Garland Set recalled by General Foam Plastic Corp. due to fire hazard sold exclusively at True Value Hardware Stores December 24, 2011 - The U.S. Consumer Product Safety Commission, in cooperation with General Foam Plastic Corp., of Norfolk, Va, today announced a voluntary recall of the Brookfield Entry Way Tree Sets. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product. The battery box that power lights on the artificial wreath can overheat, posing a fire hazard. The firm has received 12 reports of battery boxes overheating and being warm to the touch after batteries were installed. No injuries have been reported.
