The oldest building still in use in the world is the Pantheon in Rome. It was built in 117 AD, nearly 2000 years old. It must look a bit odd when it was first completed. The building is round and has an enormous dome on top. Looking at its exterior now it may not look much, but it is still one of the most admired buildings in the world, mainly because of its age and the construction technique used in the building. I wonder if any of our iconic buildings today would survive as long. Would they still appear as aesthetically pleasing as they are now? The Guggenheim Museum Bilbao is such an iconic building today. It has become the landmark of the city of Bilbao. Just like the Tower Bridge to London, the Statue of Liberty to New York, and the Eiffel Tower to Paris. In this sense the Guggenheim Museum of Bilbao is a big success. Nobody can take that away. Not only has the building shown us the use of new construction technique and the use of new material, it was also completed on time and within budget. I was not sure how to look at the building before my visit. But after looking at it, I think it is a great building and a piece of architectural art.

The terms “mainstreaming” and “inclusion” are frequently associated with educational settings or work placements. When we require the care of a health professional, we all are patients and often excluded from the direction of the process of care. The literal meaning of the word "patient" is to be passive. In a world of inclusion, empowerment and self-determination this role needs to change. The presentation discusses the consequences of this paradigm shift for the inclusion of patients in the areas of diagnostics, treatment/interventions and research.

Please write the journals in a Word document using APA format. Provide a 3-4 page response (i.e., double-spaced, plus title page and references). You may wish to talk about your, 'learning edge' or the point at which you become less comfortable with the idea of working with Indigenous children and communities. What do you know now, and what do you want to learn? Journals allow the instructor to witness the learning and thought process of each student. Some of the course materials and historical information may be difficult to process and students may need support and instructor feedback from time to time. It would be ideal to keep a journal notes every week or each time you review course material and reflect on this course: "Supporting Indigenous Infants and Young Children within the Context of Their Communities" and what this means to you now, during and near the end of the course. Students are asked to submit two personal Journals documenting their learning and authentic reflections per week in the 5 week summary. In addition, students are encouraged to document the struggles, interests, curiosities and other ideas experienced throughout the course. Journal writings should document the student’s learning, areas of struggle, challenges, interests and/or passions. You may want to consider including personal reactions or responses to some of the course content and reading or resource material. Considering the following questions: · What reactions do you experience in your body? In your mind? · What helps you to explore these topics in greater depth and integrate the material into your existing repertoire of knowledge? It may be helpful to acknowledge your, 'working edge' at the start of your Journal writing. A 'working edge' is the point where you become less comfortable with your knowledge of working with Indigenous families in an early childhood educational context. For many decades, counselors and counseling psychologists have been concerned with the relationship between individuals’ mental health and the social milieus in which people live. As the racial and ethnic diversity of the United States con- tinues to increase, the need for mental health professionals to tailor their mental health services to the needs of various cultural populations has become more germane (Constantine, Kindaichi, Arorash, Donnelly, & Jung, 2002). In particular, the growing recognition of the negative consequences of oppression in the lives of people of color has been crucial in helping many counselors and counseling psychologists to identify effective interventions to address such issues and to work more broadly to effect social change (Hage, 2003; Vera & Speight, 2003). Such awareness and actions have paralleled the emergence of the multicultural competence movement (Arredondo & Perez, 2003; Sue et al., 1982). Multicultural competence generally is defined as the extent to which counselors possess appropriate levels of self-awareness, knowledge, and skills in working with individuals from diverse cultural backgrounds (Arredondo et al., 1996; Sue, Arredondo, & McDavis, 1992). In particular, self-awareness entails being cognizant of one’s attitudes, beliefs, and values regarding race, ethnicity, and culture, along with one’s awareness of the sociopolitical relevance of cultural group membership in terms of issues of cultural privilege, discrimination, and oppression. The knowledge dimension of multicultural competence refers to information one has about various worldview orientations, histories of oppression endured by marginalized populations, and culture-specific values that influence the subjective and collective experi- ences of marginalized populations. The skills component of multicultural competence involves the ability to draw from an existing fund of cultural knowledge to design mental health interventions that are relevant to marginalized populations. In many respects, multicultural competence has become inextricably linked to counselors’ and counseling psychologists’ ability to commit to and actualize an agenda of social justice (Kiselica & Robinson, 2001). Social justice reflects a fundamental valuing of fairness and equity in resources, rights, and treatment for marginalized individuals and groups of people who do not share equal power in society because of their immigration, racial, ethnic, age, socioeconomic, religious heritage, physical ability, or sexual orientation status groups (Fondacaro & Weinberg, 2002; Prilleltensky & Nelson, 1997). Thoracotomy is the surgical procedure with an incision made to access the pleural space and the contents of the thoracic cavity. Given the structures to be accessed with in the cavity, different incisions have been established to easy the procedure and these incisions qualify the types of thoracotomies to be studied hereunder. PRE OP OPTIMIZATION FOR SPECIFIC FACTORS The outcome of surgical procedures is not measured only by clinical end points but also shorter stays and lower costs. Patients’ discharge is delayed commonly due to inadequate pain relief, infection, arrhythmias, prolonged air leak and debility. Many complications that occur from thoracic operations can be anticipated. An aggressive preoperative work up mitigates morbidity and shortens convalescence. APPROACH CONSIDERATIONS There are about three principles that can guides the choice of the thoracotomy incision to be used I. Adequate exposure must be achieved. The choice of incision is aided by a thorough understanding of the surface anatomy and a comprehensive review of the radiographic images that are obtained preoperatively. II. Chest-wall function and appearance should be preserved to the extent possible. This principle include non-spreading video-assisted thoracoscopic surgery (VATS) procedures, muscle-sparing techniques, avoidance of excessive rib retraction, and rib preservation when possible. III. The third principle is that closure must be meticulous and appropriate. Strict layered closure is the rule for thoracic surgical incisions. Every effort should be made to approximate the individual divided chest-wall muscles in appropriate layers; otherwise, a significant delay in the recovery of range of motion (ROM) may result. Care must be taken to avoid over approximating the ribs and to prevent an override; this will help minimize postoperative pain. POST-OPERATIVE CARE AND MONITORING
