SILS – Single Incision Laparoscopy Surgery involves making a single incision in the belly button to perform minimally invasive surgery using a laparoscope. It has advantages over traditional open surgery and laparoscopic surgery such as less scarring and faster recovery. The surgery allows removal of organs like the kidney or repair of the urinary tract through the single incision and additional small incisions around the belly button.

Single port is used for incision in belly in this surgery of
minimally invasive nature.
The surgery is done by means of laparoscope under
general anaesthesia
Main incision is made through navel with 3-4 additional
small incisions (ports) around it, which could be used to
remove a kidney or to repair the urinary tract.
Advantages: Little to no scarring and may reduce
complications that commonly occur after traditional
open surgery and even laparoscopic abdominal surgery.
Patients are reporting less discomfort and faster
recovery compared with those undergoing traditional
laparoscopy.

LIFE AFTER SURGERY
Diet : The modifications made to your gastrointestinal tract will
require permanent changes in your eating habits that must be
adhered to for successful weight loss. Most important is that you
strictly follow your surgeon's recommended guidelines.
Birth control and pregnancy : It is strongly advised that women
of childbearing age use the most effective forms of birth control
during the first 16 to 24 months after Bariatric Surgery. However
obese female with infertility gets significantly increased c h a n c
e s o f g e t ti n g pregnant after weight loss surgery.
Long-term follow-up : Over time, you will need periodic checks
for anaemia (low red blood cell count) and vitamin B12, folate
and iron levels. Follow-up tests will initially be conducted every 3
to 6 months, or as needed and then every 1 to 2 years. Follow-up
care is recommended for life.
Life Long Commitment: : To maintain weight loss after the
surgery, you have to continue to eat less, change your dietary
habits and increase physical activity. Weight reduction surgery
helps you in changing your food behaviour by giving early satiety
after a small meal and providing prolonged feeling of fullness so
that you can adhere to the dietary control. It also makes adhering
to the physical activity much easier. No body else than an obese
person can understand the different problems of performing
physical activity. Weight reduction with surgery certainly helps in
following the diet and exercise. what it does not do is change the
behaviour pattern, so it is very important for patients to
understand that life long commitment from their side is the most
important aspect of the management of obesity.
Post operative instruction and information :
1. After the surgery, you may be kept in the Surgical ICU
overnight.
2. You will be sleepy immediately after the operation due to
sedation given during anaesthesia and during the recovery
period.
3. You may have some discomfort when coming out of the
effect of sedation. It gradually reduces to a tolerable level
within a few minutes without any painkiller injections. You
should try to avoid these drugs because they produce
drowsiness and you may sleep for a longer period which is
not desirable.
4. You may feel nauseated in the immediate post-operation
period. In this case, you must inform the nurse so that
appropriate medication may be given to prevent vomitting,
as it is not desirable.
5. You must try to move your limbs immediately after the
surgery. You will then be gradually mobilised during the day
following surgery. Your catheter shall be removed and you
will be free to use the washroom on your own.
6. The is no restriction on your physical activity. You are
allowed to walk as soon as you are shifted to your room. You
are free to climb stairs. However you must avoid lifting
heavy weight for 4-6 weeks following surgery.
7. You shall be discharged once your oral intake of fluids
reaches 1.5-2 litres per day.
8. There will be a pipe attached to your abdomen which will be
removed during your follow-up appointment.
9. Your follow-up appointment is usually 1 week after
discharge from the hospital.
We wish you a speedy recovery...

OBESITY
SURGERY
Bilio-pancreatic diversion with duodenal switch
Procedure type - Malabsorptive
Description
It causes high degree of malabsorption to limit the food
intake
Stapling is used to bring the stomach to its original size
The majority of the small intenstine is bypased causing
nearly complete malabsorption of food.
Results
Patients have lost excess weight upto 74 % in one year,
78 % in two years, 81 % in three years, 84 % in four years
and 91 % in five years.
Causes less restriction of food consumption than other
procedures
Provides highest levels of malabsorption
Generally, priority is given to Gastric Sleeve / Bypass
Surgery
Type-II Diabetes
Surgical treatment of diabetes is now possible. 85 % to 95 %
of diabetic patients, who were suffering from obesity, had a
complete cure of diabetes after operation
Is type-II Diabetes an operatable disease?
In most cases, diabetes and obesity co-exist. Approximately
80 % of Diabetics are overweight. Initially, Bariatric and
Obesity surgery procedures were used to reduce obesity but
not it is effective for type-II diabetes also. Some gastric bypass
procedures, help remove and reduce type-II diabetes.
A surgical solution is an important innovation for patients
suffering from Obesity and Diabetes. Bariatric Surgeons from
around the world were surprised to know that within 24 to 48
hours after surgery, type-II diabetes of patients was cured
complete.

Obesity is a life-long progressive, life threatening condition
marked by the excess accumulation of body fat, which can
significantly reduce life expectancy. When weight reaches
extreme levels, it is called MORBID OBESITY and is a chronic
condition with numerous medical, psychological and social
consequences.
Finding out if you are obese is simple; you calculate your Body
Mass Index or BMI. Obesity is also defined by waist size - over
90 cm in males and over 80 cm in females.
How can Obesity affect you?
Obesity is rarely just a physiological problem . This disorder
has far reaching effects; physiological, psychological and
social. On the physical front, obesity can lead to several
complications that could be fatal or incapacitating if left
untreated.
Some of the most common diseases that are directly and
indirectly linked to obesity are:
Type II Diabetes
Heart Disease
Hypertension
Hypercholesterolemia
Liver and kidney disorders
Joint Pain/Osteoarthritis/Gout
Respiratory disorders
Gastro-esophageal reflux disease
Infertility
Certain cancers
Depression and anxiety disorders
Sleep Apnea
How to measure Obesity?
Obesity is determined by calculating body mass index
(BMI),which is calculated by
the weight in kilograms
divided byheight in meters
squared.
How to manage Obesity?
INTRAGASTRIC AIR BALOON
Obesity can be treated. But it involves more than just
swallowing a pill or sticking to a diet. It needs a
multidisciplinary assessment involving a coordinated
approach by different specialists such as dietician,
endocrinologist, psychologist, internist, cardiologist, family
doctor, bariatric surgeon.
What is Bariatric (Obesity) Surgery?
Bariatric (Obesity) surgery, or weight loss surgery, includes a
variety of procedures performed on people who are obese.
Weight loss is achieved by reducing the size of the stomach
through removal of a portion of the stomach (sleeve
gastrectomy) or by resecting and re-routing the small
intestines to a small stomach pouch (gastric bypass surgery).
Why should I consider Weight Loss Surgery ?
Because it works! Surgical treatment for obesity is the ONLY
treatment that reliably produces significant and sustained
weight loss. People with clinically severe obesity are at great
risk for developing many associated medical conditions.
Research has shown that surgical treatment results in
significant weight loss and improvement in most problems
associated with obesity. Surgical treatment for clinically
severe obesity has been endorsed by the National Institutes
of Health, the World Health Organization, the American Heart
Association, the American Dietetic Association, and the
American Obesity Association.
Does my weight alone justify such extreme measures?
Yes. Morbid obesity is an independent risk factor for
premature death, with the risk rising as the BMI increases.
People with a BMI of 30 have a relative risk of dying early that
is 1.3 times greater than normal weight individuals. By the
time the BMI is 40, the risk is close to 3 times as great.
An air balloon is implanted throughthe endoscopic route in
only around30 minutes without the need tohospitalize the
patient. Thisprocedure of implanting is nonsurgical and
minimally invasive thusreducing post implantation
nausea, vomiting and the feeling of pain.
How it works to help you lose weight?
Intragastric Air Balloon is filled inside the stomach to
createshort term and rapid weight loss. It promotes a
sensation ofhaving eaten too muchafter a little meal andthus
loses weight at afast rate. This balloonis temporary and hasto
be extracted aftersix months.
SLEEVE GASTRECTOMY
Obesity Management
The Vertical Sleeve Gastrectomy (VSG) generates weight
losssolely through gastric restriction (reduced stomach
volume). In VSG approximately 2/3rd of the stomach is stapled
offalong its greater curvature, leaving behind 1/3rd
stomachalong lesser curvature, which is roughly the size and
shape ofa Banana or Sleeve.
This operation does not involve any “rerouting”
orreconnecting of the intestines. Hence it is technically
asimpler operation than the gastric bypass.
How it works to help you lose weight?
Food absorption is normal, with no malabsorption. It isknown
to reduce hunger because Ghrelin (hungerstimulating
hormone) producing part of stomach isremoved. Provides
satiety with small amount of food andunlike gastric band &
gastric bypass, patients feels full withliquids as well.
Under this procedure, a small stomach pouch is created
andsection of the small intestine is directly attached to
thepouch. By creating a smaller stomach pouch, a
GastricBypass limits the amount of food that can be eaten at
onetime, so you feel full sooner and stay full longer. It also
causesyour body to absorb fewer calories.
How it works to help you lose weight?
As you eat less food and absorb fewer calories, your body
willstop storing excess calories and start using its fat supply
forenergy.

Gastric Bypass Roux-en-y
Procedure type – combined Restrictive / Malabsorptive
Description
A small pouch is created in the upper stomach which
restricts the amount of food consumption
With the help of stapling a portion of small intestine is
bypassed thus delays the mixing of food with digestive
juices thus calorie is not obsorbed completely
Results
Average 77 % of excess body weight is lost in one year
after surgery
A study shows that after 10 to 14 years, patients have
maintained 60 % of excess body weight loss. A study of
500 patients showed that 96 % of physical problems
were improved and resolved in some cases which mainly
includes back pain, sleep apnea, high blood pressure,
type-II diabetes and depression.
In most cases, patients report an early sense of fullness
even by taking little food so that there will be no desire to
take more food.
