possible. By answering the following survey, you will help us serve you better. Please take a few
moments to answer these questions about the site. Remember to read the sending instructions

Sending Instructions:
OPTION 1:
• Click Done
• After clicking “DONE”, you will be prompted to save the file. Rename the file in this
format Reply_Form_”Branch or Dept name” (i.e. Reply_Form_CCD, Reply_Form_AYALA)
and save to Desktop.
• A prompt will appear asking you that the document is attempting to open the link

• Please send a message with the Subject: Intranet Survey Form – Reply
• Attach the renamed file and email to intrafeedback@sss.gov.ph
OPTION 2:
• Print the duly accomplished survey form and send via fax to 924‐7328.
