CC: ปวดข้อเท้าขวา 4 ชั่วโมงก่อนมาโรงพยาบาล
PI: 4 ชั่วโมงก่อนมาพบแพทย์ขณะผู้ป่วยกาลังจะออกจากบ้าน ถูกประตูรั้วล้มทับข้อ
เท้าขวาและล้มลง ข้อเท้าขวาบิดเข้าข้างใน มีอาการปวดบวม ขยับข้อเท้าขวาไม่ได้
ไม่สลบ จาเหตุการณ์ได้ ญาติจึงนาส่งโรงพยาบาล

3. Primary survey
A: can speak, no cervical tenderness
B: normal breath sound equal both lung , no chest wall contrusion , Oxygen
sat. 99%
C: BP: 122/81 mmHg, HR: 80 bpm, no active bleeding
D: E4V5M6, pupil 3 mm RTLBE
E: Right ankle marked swelling with inversion deformity, tender at lateral
and medial malleolus, dorsalis pedis 2+ , sensory intact

8. Trimalleolar fracture
(Supination-Eversion Rotaion, Type- B3)

14. 1. Nonoperative
-isolated nondisplaced medial malleolus fracture
-isolated lateral malleolus fracture with < 3mm displacement
2. Operative
-Bimalleolar fracture
-Trimalleolar fracture
-Syndesmosis injury (Tibiofibular clear space >6mm)
-Deltoid ligament injury (Medial clear space > 4mm)
