5. Plasmozytombildgebung - Andrei Dumitrescu - 25.10.2010
Altersverteilung
Relevant bei der Wahl eines diagnostischen Verfahrens unter
Berücksichtigung der Strahlenexposition
6. Plasmozytombildgebung - Andrei Dumitrescu - 25.10.2010
Aber:
• Die meisten Patienten bekommen mindestens jährliche
Verlaufskontrollen über ca. 4-5 Jahre
• Kumulation der Strahlenbelastung
• Dosis Röntgen (Pariser Schema): durchschnittlich 2,1 mSv
• Dosis CT (low dose): durchschnittlich 4-6 mSv je nach Protokoll
• Es wird geschätzt, dass
Pro 10 mSv Strahlendosis einer in 5000 Patienten
eine strahlenassozierte Krebserkrankung entwickelt

Empfehlungen zur Senkung der diagnostischen
Strahlenbelastung
• „There is direct evidence from epidemiologic studies that the organ
doses corresponding to a common CT study (two or three scans,
resulting in a dose in the range of 30 to 90 mSv) result in an increased
risk of cancer. The evidence is reasonably convincing for adults and
very convincing for children.“
• „About 1,5 - 2% of all cancers in the United States may be attributable
to the radiation from CT studies.“
8. Plasmozytombildgebung - Andrei Dumitrescu - 25.10.2010
Empfehlungen zur Senkung der diagnostischen
Strahlenbelastung
„Given recent reports of increased risks of radiation-induced cancer
from low-level radiation doses similar to those used in diagnostic
imaging and coupled with the fact that patients with monoclonal
gammopathy of uncertain significance (MGUS) should not be
unnecessarily exposed to excessive levels of radiation, the (as low as
is reasonably achievable) ALARA principle should be rigorously
employed, now more than ever.“
9. Plasmozytombildgebung - Andrei Dumitrescu - 25.10.2010
Technischer und personeller Aufwand
• Untersuchungszeit (MTRA):
Pariser Schema: ~ 28 Min. CT: ~ 9,8 Min.
• Aber: „effects of increased interpretation times for
radiologists were not assessed“
• Zum Vergleich:
Pariser Schema: ca. 13 Bilder CT: ca. 1700 Bilder

Upstaging – immer im Sinne des Patienten?
• „due to higher sensitivity, patients will be upstaged and
treatment in this group of patients will start in an earlier
phase of the disease“
• „Whole-body CT resulted in restaging in 25 out of 39
instances, 20 of which were upstaged“
