The prevalence of eczema at 1 yr in the probiotic group was significantly lower than in the placebo group (18.2% vs. 40.0%, p=0.048). The cumulative incidence of eczema during the first 12 months was reduced significantly in probiotic group (36.4% vs. 62.9%, p=0.029); however, there was no difference in serum total IgE level or the sensitization against food allergens between the two groups. Conclusion : Prenatal and postnatal supplementation with a mixture of B. bifidum BGN4, B.lactis AD011, and L. acidophilus AD031 is an effective approach in preventing the development of eczema in infants at high risk of allergy during the first year of life.

CONCLUSION: We found that supplementation with L rhamnosus, but not B animalis subsp lactis, substantially reduced the cumulative prevalence of eczema, but not atopy, by 2 years. Understanding how Lactobacilli act to prevent eczema requires further investigation.

Further work is needed to determine whether timing of supplementation, dose and probiotic strain are important considerations.

OBJECTIVE. primary prevention of allergic disease in high-risk children by pre- and postnatal supplementation of selected probiotic bacteria METHODS. double-blind, randomized, placebo-controlled trial, a mixture of probiotic bacteria selected by in-vitro experiments (Bifidobacterium bifidum, Bifidobacterium lactis, and Lactococcus lactis, was prenatally administered to mothers of high-risk children and to their offspring for the first 12 months of life. Allergy. 2009 Sep;64(9):1349-58 CONCLUSIONS : This particular combination of probiotic bacteria shows a preventive effect on the incidence of eczema in high-risk children, which seems to be sustained during the first 2 years of life.

Pediatr Allergy Immunol. 2010 Feb;21:67-73 In infants with high faecal IgA concentration at the age of 6 months, the risk of having any allergic disease before the age of 2 yr tended to reduce [odds ratio (OR: 0.52)] and the risk for any IgE-associated (atopic) disease reduced significantly (OR: 0.49) Probiotics tended to augment faecal IgA (p = 0.085)….. High intestinal IgA in early life associates with minimal intestinal inflammation and indicates reduced risk for IgE-associated allergic diseases.

43. OBJECTIVE. The purpose of this trial was to study the preventive effect of the probiotic Lactobacillus GG on the development of atopic dermatitis. Pediatrics 2008;121;e850-e856 METHODS. 105 pregnant women from families with 1 member with an atopic disease were randomly assigned to receive Lactobacillus GG twice daily or placebo. The supplementation period started 4 to 6 weeks before expected delivery, followed by a postnatal period of 6 months. The primary end point was the occurrence of atopic dermatitis at the age of 2 years. Secondary outcomes were severity of atopic dermatitis, recurrent episodes of wheezing bronchitis, and allergic sensitization at the age of 2 years. CONCLUSIONS. Supplementation with Lactobacillus GG during pregnancy and early infancy neither reduced the incidence of atopic dermatitis nor altered the severity of atopic dermatitis in affected children but was associated with an increased rate of recurrent episodes of wheezing bronchitis. Therefore, Lactobacillus GG cannot be generally recommended for primary prevention.

SOLO il 2 % degli studi clinici per testare i farmaci a scopo pediatrico, si basa su Comitati di Controllo Indipendenti (non legati ad industrie farmaceutiche), per valutare l’eventuale presenza di reazioni avverse. Questo e’ quanto emerge da un’analisi, effettuata dall’Universita’ di Nottingham (UK) ed apparsa sulla rivista Acta Pediatrica di 739 trial internazionali effettuati fra il 1996 ed il 2002, ed alcuni test comprendevano anche adulti assieme ai bambini… Mentre il 74 % degli studi pubblicati descriveva il modo in cui era stato condotto il monitoraggio sulla sicurezza, solo 13 su 739 avevano comitati composti da membri NON arruolati dalle aziende farmaceutiche !
52. Un editoriale, pubblicato su Nature, intitolato "I cattivi comportamenti degli scienziati", rivela allarmanti notizie sul mondo scientifico. L'inchiesta e' stata condotta dai National Institutes of Health su 3.600 ricercatori e 4.160 borsisti. Circa il 50% ha risposto ad una serie di domande che ha messo in luce comportamenti preoccupanti: per esempio, lo 0,3% ha affermato di avere falsificato i dati o di averli inventati e il 15,5% ha modificato la linea e la metodologia di ricerca per accontentare chi finanziava lo studio . In totale, circa il 33% di coloro che hanno risposto alle domande ha avuto una condotta sleale .

Secondo alcune valutazioni circa i 3/4 delle ricerche biomediche che vengono condotte attualmente nelle strutture assistenziali (e in parte minore anche in quelle non-cliniche), sarebbero sostenute dall’industria. Una indagine effettuata negli Stati Uniti per iniziativa della Health Partners Research Foundation di Minneapolis, e condotta su 3247 ricercatori ha messo in luce come un numero rilevante degli intervistati abbia ammesso di aver proposto interpretazioni non corrette dei dati ottenuti e come circa il 15% abbia confessato di aver modificato il proprio studio su pressione degli sponsor commerciali (Corriere della Sera 14-6-2005). SICUREZZA DEI BATTERI PROBIOTICI Sempre ai fini della determinazione della sicurezza si ribadisce la necessità di una identificazione tassonomica su base genetica , almeno a livello di specie, e la necessità di mantenere inalterata la presenza dei ceppi nei prodotti. L’introduzione in un prodotto di nuovi ceppi microbici, sia pure appartenenti alla stessa specie, richiederà una nuova valutazione della sicurezza e dell’efficacia.

L’evoluzione delle conoscenze relative alle attività probiotiche è rapidissima, testimoniata dal numero di nuove recensioni che appaiono nelle banche dati tipo Medline. ………., in particolare tutte le 7 meta analisi sopra citate hanno evidenziato una positiva azione dei probiotici nel ridurre i tempi di recupero da diarrea indotta da terapie con antibiotici. Diversi di questi documenti suggeriscono l’importanza della dose somministrata ai fini di ottenere un effetto positivo; tale importanza è anche implicita nella definizione stessa di probiotico.
