Guideline for blood products transfusion in DHF
PLATIELETS TRANSFUSION
Indications
1. Platelet < 10,000 /cumm
2. Platelet < 20,000 /cumm with significant coagulopathy
3. Platelet < 50,000 /cumm prior to invasive procedure
4. Platelet < 50,000 /cumm with clinical significant bleeding i.e. hematemesis, melena, epistaxis not
response to local treatment, pulmonary hemorrhage, bleeding from wound, hematoma
Dosage 0.1 unit of random donors platelet/kg/dose
FRESH FROZEN PLASMA TRANSFUSION
Indications
1. Use as colloidal replacement fluid after failure of crystalloid replacement for > 2 hr, dosage not
exceed 10 ml/kg/hr (max 40 ml/kg/day)
2. Use for correction of coagulopathy: liver coagulopathy, DIC, dosage 10-20 ml/kg/dose
q 6-12 hr (max 40 ml/kg/day)
3. Use for plasmapheresis in hepatic failure
4. Use in combination with PRC for blood exchange transfusion or replacement of blood loss
Contraindication
1. Uncontrolled pulmonary edema
2. Congestive heart failure
PRC TRANSFUSION
Indications
1. Hct < 35 % during shock stage
2. Hct declines > 5% in less than 4 hr or > 8% in less than 24 hr
Hct prior to transfusion should not exceed 40%
3. Replacement therapy for massive bleeding
4. Use with FFP for blood exchange transfusion
Dosage 10 ml/kg/dose for correction of anemia or drop of Hct
Replacement of continuous blood loss cc/cc
Other Coagulation factors replacement
Factor VIIa (Novo seven)*
Indications
1. Emergency life-threatening bleeding i.e. cerebral hemorrhage
2. Significant uncontrolled bleeding after maximum platelet (0.4 unit of random donors
platelet/kg/day) and maximum FFP transfusion (>40 ml/kg/day)
3. Significant life-threatening bleeding and unavailable platelet and/or FFP.
Notify Hematology attending before prescribe this product!!

Management of Hepatic Encephalopathy

การดูแลรักษาผูปวย DHF ที่มีภาวะไตวายเฉียบพลัน
1. การใหสารน้ํา
ตองจํากัดการใหสารน้ําในผูปวยที่มี oliguria หรือ anuria ที่ไมไดเกิดจากสาเหตุทาง prerenal (ยกเวน heart
failure) ซึ่งสวนใหญเกิดจาก plasma leakage โดยปริมาณที่ไดรับตอวัน ควรเทากับ ผลรวมของ insensible loss
(300-400 ml/m2/day), urine output และ ongoing loss อื่นๆ (ถามี) การปรับเปลี่ยนปริมาณของสารน้ํา ที่ให
อาศัยคา central venous pressure (ถามี) การเปลี่ยนแปลงของน้ําหนักตัว การตรวจรางกาย ปริมาณสารน้ํา ที่
ไดรับและสูญเสีย รวมทั้งปสสาวะในชวงเวลาที่ผานมา
2. การแกไขภาวะไมสมดุลของแรธาตุและกรด-ดาง
2.1 Hyponatremia ที่เกิดจาก fluid overload มักจะดีขึ้นภายหลังจํากัดการใหสารน้ํา อยางไรก็ตาม ในกรณีที่
serum Na < 120 mEq/L และ/หรือมีอาการจากภาวะ hyponatremia จําเปนตองแกไขโดยเรงดวน เพื่อใหระดับ
serum Na มากกวา 125 mEq/L โดยให 3% NaCl ซึ่งประกอบดวย Na 513 mEq/L ทางหลอดเลือดดําอยางชาๆ
ปริมาณ 3%NaCl ที่ให (mL) = [0.6 X wt (kg) X (125 – actual serum Na)] X [1000/513]
2.2 Hyperkalemia
ถา serum K+ 5-6 mEq/L : จํากัด potassium intake <0.5 mEq/kg/day และให furosemide
ถา serum K+ 6-6.5 mEq/L : ใหการรักษาขางตนรวมกับให NaHCO3, b-agonist, kayexalate
ถา serum K+ 6.5 mEq/L : ใหการรักษาขางตนรวมกับให Ca gluconate, glucose and insulin; อาจ

2.3 Acidosis
พิจารณาให NaHCO3 0.5-1.0 mEq/kg/dose ทางหลอดเลือดดําในเวลา 1 ชม. ควรแกไขภาวะ
hypocalcemia กอนใหการรักษาดวย NaHCO3 และควรระวังปญหา CO2 retention ในผูปวยที่มี respiratory
failure รวมทั้งภาวะน้ําเกินที่อาจเกิดขึ้นในขณะไดรับ NaHCO3
2.4 Calcium & phosphate
ผูปวยที่มีภาวะ hyperphosphatemia ควรไดรับอาหารที่มี phosphate ต่ํา และ phosphate binder ไดแก
oral calcium carbonate พรอมมื้ออาหาร ขนาดเริ่มตน 30-50 mgCa/kg/day สวนกรณี hypocalcemia อาจ
พิจารณาให 10% calcium gluconate ในขนาด 1 ml/kg (max 10 ml)
3. หลักการใหยาตางๆ
หลีกเลี่ยง nephrotoxic drugs และปรับขนาดของยาใหเหมาะสมกับ renal function ในขณะนั้น โดยคํานวณ
estimated GFR (ml/min) = [k x height (cm)] / serum creatinine (mg/dl) โดยที่ k = 0.45 ในอายุ <1 ป และ
0.55 ในอายุ 2-12 ป อยางไรก็ตามการคํานวณดังกลาวอาจไดคา GFR มากกวาความเปนจริง
4. การใหสารอาหาร
เนื่องจากผูปวยอยูในภาวะ high catabolic การใหสารอาหารที่เหมาะสมนันมีความจําเปนอยางยิ่ง อาหารที่

ไดรบควรเปนอาหารที่ประกอบดวย high biologic value protein (essential branch-chain amino acid), low

phosphorus และ low potassium ผูปวยควรไดรบแคลอรี่อยางนอยเทากับ maintenance calories ปริมาณ protein
ั
ที่ใหประมาณ 1-2 gm/kg/day ถาไมสามารถใหอาหารทางปากหรือสาย NG พิจารณาให parentenal nutrition
ผานทาง central line
5. Renal Replacement Therapy (RRT)
เนื่องจากผูปวยเหลานี้มีความเสี่ยงตอภาวะเลือดออกงาย ใหปรึกษาแพทยผูชานาญในการทํา RRT โดย
ํ
พิจารณาในผูปวยซึ่งมีภาวะตอไปนี้
5.1 ปสสาวะออกนอย เปนอุปสรรคตอการใหยาและสารอาหาร
5.2 ภาวะน้ําเกิน จนมีความเสี่ยงตอภาวะหัวใจวาย
5.3 ภาวะ hyperkalemia ซึ่งการรักษาทางยาไมไดผล
5.4 ภาวะ severe metabolic acidosis ซึ่งการรักษาทางยาไมไดผล
5.5 ระดับ BUN สูง (>50-150 mg/dl) โดยเฉพาะรวมกับอาการและอาการแสดงของ uremia เชน คลื่นไส
อาเจียน ซึม ชัก เยื่อหุมหัวใจอักเสบ
