What is ODK?
• Open Data Kit (ODK) is a free and open-source
set of tools which help organizations author, field,
and manage mobile data collection solutions.
• It supports a wide range of question and answer types,
and is designed to work well without network
connectivity.

1. Go to the HOME SCREEN
2. Find the ODK COLLECT program (It has an
icon that looks like this) and tap on it.
3. You should see the screen on the right
4. There are 5 options:
● Fill Blank Form
● Edit Saved Form
● Send Finalized Form
● Get Blank Form
● Delete Saved Form

● Use https://-------- to configure ODK
Collect server settings
● Enter your Ona username and password
to download forms to your device and
submit data

● You are about to start a new form.
● This is the list of forms on the phone.
● To choose a form, touch the title of the
form
Select Fill Blank Form;

“Training Practice
Survey” to practice
using ODK Collect.
● You will then see
the title page (on
the right)

1. Read the first question.
2. Type in your name using the keyboard at
the bottom of the screen.
3. Practice touching the keys to write your
name.
4. When you done, swipe the screen to go
backward and forward
For our practice form, answer all
the questions about yourself.

1. For the second question, try swiping with no
answer
2. You will see this message on the screen.
3. Make sure to answer all the questions!

number answers.
2. The keyboard will change to numbers.
3. For Question #3, the answer is 54.

1. For Question #4, there is no limit to the
number of siblings you can have!
2. Type in the number of brothers and
sisters you have.
3. Swipe from right to left to go to Question
#5.

1. Question #5 is a multiple
choice question. The answer
will be YES or NO.
2. Choose YES by touching the
button to the left of the word
YES.
3. The button will change color
from white to blue.

1. Notice that if you choose YES,
the next question is #7.
2. Now go back to Question #5 and
choose NO.
3. What happens?

1. After you choose NO for Question #5, the next question is #6
2. For some of the questions in the form, the answer will determine what the
next question is.
3. Sometimes, the numbers are not in order, but that is ok!

1. In Questions #5 and #6, you can only choose ONE
answer.
2. For Question #7, you can choose MORE THAN ONE
answer
3. Tap on the box next to the fruits and/or vegetables that
you have eaten in the past week.
4. Is something missing from the list? Click on the box next
to OTHER.

1. Question #8 asks you to take a photo.
2. Tap on the TAKE PICTURE button.

3. The next screen functions like a camera.
The camera lens is on the back of the
phone.
4. Move the camera around until you are
happy with your photo.
5. When you are ready to take the photo,
click on the white camera button on the
right hand side of the screen.

6. Tap on if you are happy with the
photo. If you want to take a different
photo, tap .

7. If you want to replace the photo, tap on
TAKE PICTURE
8. If you are happy with the photo swipe the
screen from right to left to move to the next
question.

1. Go outside the building
2. Tap on RECORD LOCATION
3. Wait for the GPS to get as close as possible to the
actual location (5m if possible)
4. Tap on the second RECORD LOCATION
5. Go back to the HOME SCREEN and disable GPS to
save battery. Return to ODK Collect form.
6. Swipe the screen from right to left to move to the next
question

1. For Question #11, you will need to select
today’s date
2. Scroll to select Month and Year, then
select Day from the calendar.

Congratulations! You have finished your
first ODK Collect form!
1. The form title should already appear as the
form name.
2. At the end of the form, make sure you tap on
the checkbox to Mark the form as finalized.
3. Tap on Save Form and Exit.
Remember, the form must be marked as finalized
to be sent to the server!

1. Incomplete saving yields bad data and
should only be done WHEN YOU
ABSOLUTELY MUST, with the permission
of your team leader.
2. RULE:
If you have to uncheck the box, you must
write a reason.
Only with Supervisor Permission!

1. Always make sure you mark the form as finalized before submitting.
2. Do not leave any questions blank
