14. NONSTEROIDAL ANTIINFLAMMATORY DRUGS OR ACETAMINOPHEN FOR
OSTEOARTHRITIS OF THE HIP OR KNEE?
A SYSTEMATIC REVIEW OF EVIDENCE AND GUIDELINES

CONCLUSION:
Acetaminophen is often effective in OA and is associated with fewer adverse
reactions than NSAID. Available evidence supports the recommendations of recent
guidelines to use acetaminophen as initial therapy for OA in addition to
nonpharmacological interventions. Further research is needed to establish the
efficacy of NSAID or acetaminophen in relevant subgroups of patients. We agree with
guidelines that it is important that treatment is tailored to individual patients taking
into account the severity of symptoms, previous use of acetaminophen, and the
patient's knowledge, expectations, and preferences.

Effectiveness of non-steroidal anti-inflammatory drugs for the treatment of
pain in knee and hip osteoarthritis: a network meta-analysis.

Lancet. 2016 May 21;387(10033):2093-105. doi: 10.1016/S0140-6736(16)30002-2. Epub 2016 Mar 18.
INTERPRETATION:
On the basis of the available data, we see no role for single-agent paracetamol for the
treatment of patients with osteoarthritis irrespective of dose. We provide sound evidence
that diclofenac 150 mg/day is the most effective NSAID available at present, in terms of
improving both pain and function. Nevertheless, in view of the safety profile of these drugs,
physicians need to consider our results together with all known safety information when
selecting the preparation and dose for individual patients.
65. Findings from randomized controlled trials of knee
arthroplasty generally showed cold compression therapy
provides better outcomes such as pain relief than
alternative interventions. While the effects of cold and
static compression are clearly better than no treatment,
they do not appear to be directly additive.

• Reduce pain by shifting your weight off the
most damaged portion of your knee.
• Improve your ability to get around and help
increase the distance you can walk
comfortably.
• Knee braces come in a variety of designs, but
most are constructed with a combination of
rigid and flexible materials — plastic, metal or
other composite material for basic structure
and support, and synthetic rubber or moldable
foam for padding and positioning.
