Grosso problema epidemiologico; prima causa visita ambulatorio di
pancreas!
Molto comuni dai 55 anni in su
Quasi 2/3 sono IPMN (possibile lesione preneoplastica)
Ma grosso gap tra la loro incidenza e quella di tumore del pancreas!
(cisti 10%, tumore 15/100.000)… controlliamo (o operiamo) polipi
iperplastici per prevenire adenocarcinoma del colon?

nodule, and an abrupt change in the calibre of the pancreatic duct with distal pancreatic atrophy
→EUS → mucinous content → surgery (?)

Diameter ≥3 cm; thickened enhancing cyst wall; main duct size 5–9 mm, enhancing mural

positive or suspicious cytology → surgery

33. Summary: Cisti Pancreas Occasionale
Davvero molto frequenti: niente panico e «scartare» subito gli unfit a basso rischio
La diagnosi del tipo di cisti deve essere accurata e questo non sempre è semplice
Indicazioni a chirurgia da definire usando tutti gli esami a disposizione
Servirebbero RCT tra diversi approcci e andrebbe costruito e validato uno score di
rischio da applicare NON SOLO ai pazienti operati
Il FU è sicuro e va continuato finchè l’aspettativa di vita del paziente lo suggerisce

39. Pitfalls in Epidemiology Studies of CP
1. Tertiary care Centres are subject to spectrum bias and often
do not see mild cases that are difficult to diagnosed
2. Patients with very early or late disease phases might not be
seen by gastroenterologists or surgeons but by PCPs or
other specialists (diabetologists?)
3. The autoptic prevalence of CP is far higher (5-10% and 18%
in cirrhotics!)

ricorrente è frequente
2. Di fatto il progresso PA – RAP – PC è un continuum
3. Individuare presto una PC è rilevante perché il paziente
ha un rischio di morte e di eventi non pancreatici
elevato
4. Lo stato nutrizionale del paziente causa la maggior
parte di questi eventi e va valutato con attenzione e
corretto.
