those in need that are dealing with organ failure by using funds to drive

a charitable life was founded by Stephen Preusz. At the age of 29, he was
diagnosed with IGA nephropathy,orEnd Stage Renal Failure (Kidney
Failure). He began ACL as a way to providefor those enduring the same
experiencesand processesthat he went through. a charitable life acts as
a regular insurance agency except that a portion of the proceedsearned
go to charity.
3. a charitable life, Inc. (“ACL”) has developed a new employee benefits plan
called The Wellness Transformation (“The WT”), and just like most
programs, it saves payroll taxes payable by the employer and, at the same
time, can increase supplemental coverage to the employee, with almost
no change in pay, while giving the employee Choices…
• Choice of what insurance to buy based on their needs
• Choice to participate in a participation based wellness program
• Choice of supplemental benefits for employees
There is no other choice where your benefits can increase, take home
pay can increase, and employers can save money necessary to grow
their business!
Whatis ACLWellness?…
4. WhyNow?...
• Employers are looking for cost savings to offset increased expenses
and stimulate growth in this environmentof regulation and financial
turmoil.
• Huge cost shifting to employees is now at a critical tipping point.
• Medically related bankruptcies are rising at an alarming pace,
even when covered by major medical plans.
• Health Insurance alone is not getting the job done!
• Wellness is the right thing to do!
5. The Opportunity
If you asked a company owner or employer
the following question…
“If I could save you an average of $500 per
employee per year on your FICA taxes, would
you be interested?”
…how many do you think would say “YES!”?
7. The Opportunity
The next question that you may receive is…
How is this possible?
How does this work?
What’s the catch?
8. The Opportunity: How it works
The employer will sponsor a
defined contribution plan and
allow the employee to choose
their best option.

9. The Opportunity: How it works
Thereafter, each month, the
participating employee makes a
standardized contribution to his/her
WT in an amount pre-set by WT in
accordance with rules of the WT.
These contributions are deemed to be
on a pre-tax basis and excluded from
taxable income within the IRS Section
125 Rules.

10. The Opportunity: How it works
The employee’s income subject to
payroll taxes is reduced and the
WT reduces the payroll tax
obligations of the employee and
employer. The result is an
average savings of $500 per
employee annually.

11. The Opportunity: How it works
After the employee makes his pre-
set contribution to the WT, he
receives the chosen benefit which
is in the form of wellness
incentives.
