O documento discute o crescimento infantil, juvenil e de crianças, afirmando que não é constante, ocorrendo em surtos curtos e períodos de crescimento mais lento. Recomenda-se medir a altura a cada três meses no mínimo para avaliar adequadamente a velocidade de crescimento.

1. O CRESCIMENTO DE CRIANÇA, INFANTIL E JUVENIL NÃO É
CONSTANTE, SENDO CARACTERIZADO POR SURTOS CURTOS E
PERÍODOS DE CRESCIMENTO MAIS LENTOS. O INTERVALO
ENTRE AS MEDIÇÕES DEVE SER ADEQUADO A FIM DE PERMITIR
UMA AVALIAÇÃO PRECISA DA VELOCIDADE DE CRESCIMENTO.
Embora as avaliações clínicas, sintomatológicas devam ser medidas se
possível mensalmente, os
intervalos de amostragem
apropriada variam de acordo
com a idade, mas não devem
ser inferiores a três meses para
avaliação do crescimento
longitudinal ou linear durante a
infância, sendo 6 meses o
intervalo ideal ao
considerarmos uma reavaliação
mais detalhada da evolução de
todos os parâmetros que
interferem no crescimento de
criança, infantil e juvenil. O
problema da medição da taxa
de crescimento de criança com
deformidade ortopédica ou contratura é algo significativo, uma vez que
esses pacientes também podem possuir distúrbios endócrinos e/ou
nutricionais. A medida da altura dos joelhos, comprimento tibial ou do
braço correlacionam-se bem com a medição da estatura linear ereta
(0.97); assim sendo, estas aferições devem ser traduzidas para a estatura
linear total, usando-se equações de regressão linear especial, sendo
então colocadas nos gráficos de crescimento conforme protocolos
padronizados. Os dispositivos a laser usados para a medição do
comprimento tibial (joelhometria) são considerados precisos para a
avaliação do crescimento linear a curto prazo em intervalos semanais,
2. entretanto, talvez a utilização em um tempo tão curto possa não ser
uma necessidade vital ou que possa trazer algum benefício tão imediato
a não ser em casos excepcionais.
Além da estrutura ou comprimento linear, outras aferições significativas
incluem:
(1) A circunferência fronto-occipital;
(2) A envergadura horizontal dos braços (entre as falanges dos dedos
médios estendidos com o paciente de encontro a uma parede ou placa;
(3) A proporção entre o segmento superior (SS) e o segmento inferior
(SI), sendo que para este último, o SI é medido a partir do topo da sínfise
pubiana verticalmente até o solo com o paciente em posição ereta, e o
SS é determinado por meio da subtração do SI da estatura em pé (os
padrões da proporção do SS-SI). Estas aferições tornam-se importantes
nos distúrbios esqueléticos ou reprodutivos.
A estatura na posição sentada é usada em alguns estudos clínicos do
crescimento linear, principalmente para pesquisas, porém o
estadiômetro em posição sentada raramente está disponível.
3. Em resumo, podemos considerar três critérios para baixa estatura linear
patológica:
(1) A estatura mais do que 3,5 DP abaixo da média para a idade
cronológica;
(2) A taxa de crescimento mais de 2 DP abaixo da média para a idade
cronológica e
(3) A estatura mais de 2 DP abaixo da estatura linear estimada após a
correção da estatura linear parental média. Portanto, a baixa estatura
linear intra-útero, neonatal, criança, infantil, juvenil e adolescente é uma
situação que independente da patologia complexa a qual é submetida, é
um problema psicobiossocial que também compromete adultos de
forma perene.

Although the clinical, symptomatic monthly should be as far as possible,

The problem of measuring the rate of linear growth of children with
orthopedic deformities or contraction is somewhat significant, since
these patients also have endocrine and/or nutritional disorders. The
measure of the knees, tibial length or arm correlate well with the
measurement of standing height (0.97); therefore, these measurements
must be translated into the full stature using linear regression equations
of special, being then placed on growth charts according to standardized
protocols. Laser devices used for the measurement of tibial length
(kneemetry) are considered accurate for the evaluation of short-term
linear growth at weekly intervals, though perhaps for use in such a short
time may not be a vital need, or which may bring some benefit as
immediately unless exceptional cases.
Besides the structure or length other significant measurements include:
(1) The fronto-occipital circumference;
(2) The horizontal arm span (between the phalanges of the middle
fingers extended with the patient against a wall or board, and
(3) The ratio of the upper segment (US) and the lower segment (LS) for
the latter, LS is measured from the top of the pubic symphysis vertically
to the ground with the patient in upright position, and the US is
determined by subtracting the LS height to feet (standards proportion
US-LS these measurements make-was important in skeletal or
5. reproductive disorders stature in the sitting position is used in some
clinical studies of linear growth, primarily for research, but the
stadiometer in sitting position is rarely available in short stature.
We consider three criteria for pathological low stature:
(1) The linear height more than 3.5 SD below the mean for chronological
age,
(2) The linear growth rate of more than 2 SD below the mean for
chronological age and
(3) The linear height more than 2 SD below the estimated height after
correction with parental average linear height. So the short linear
stature intrautero, neonatal, juvenile, infant, children and adolescent is
a complex situation that regardless of pathology which is submitted, is a
problem that also compromises psicobiossocial adults´ perennial form.
