fatores de crescimento epitelial placentar (EGF-R) e uma redução da
afinidade destes receptores de EGF-R nas placentas de mulheres
fumantes. Pacientes hipertensas também apresentam redução na
2. quantidade de EGF–Rs, podendo ocasionar Restrição do crescimento
intrauterino (RCIU). Os níveis de EGF-fatores de crescimento epitelial no
liquido amniótico normalmente encontram-se aumentados próximo ao
termo, porém estão reduzidos em gestações complicadas por Restrição
do crescimento intrauterino (RCIU) apesar de não estarem aumentados
em crianças grandes para idade gestacional. Os níveis de EGF nas
primeiras urinas eliminadas por crianças com RCIU e macrossomia são
inferiores ao de crianças-controle. O EGF administrado em feto de
animais de grande porte (por ex. macacos) resulta em uma maturação
histológica e bioquímica de seus pulmões, ocasionando uma troca de ar
e uma diminuição da necessidade de suporte respiratório. A
concentração da Apoproteína surfactante A e a proporção lecitina-
esfingomielina encontram - se elevadas no liquido amniótico de fetos
tratados com EGF.
Surfactante pulmonar exógeno é um fármaco fundamental no
tratamento de recém-nascidos com história de síndrome de dificuldade
respiratória; é por esta razão que é importante conhecer os diferentes
tipos disponíveis no mercado, sua história e propriedades destes, bem
como informar a dosagem, componentes da droga e das indicações dos
tipos de surfactante
pulmonar que estão no
mercado. Portanto, a
terapia com surfactante
pulmonar exógeno é
amplamente utilizada no
tratamento de doença da
membrana hialina (DMH)
em recém-nascidos
prematuros que
necessitam de suporte
ventilatório. O
desenvolvimento desta
terapia tem sido um
importante passo histórico para à frente no cuidado intensivo neonatal.
Tem-se estudado o seu uso benéfico em outras doenças respiratórias do
recém-nascido, como a síndrome de aspiração de mecônio, a hemorragia
pulmonar e a broncopneumonia. As apoproteínas surfactantes são
3. quatro SP-A, SP-B, SP-C e SP-D. A SP-A e SP-D são hidrofílicos e SP-B e SP-
C são hidrofóbicos. A SP-A e SP-D desempenham um papel importante
na defesa contra agentes patogênicos inalados e a SP-A que também
tem um papel regulador na formação da monocamada que reduz a
tensão superficial. As proteínas hidrofóbicas são necessárias para
melhorar a disseminação de fosfolipídios nos espaços aéreos. A SP-B
promove a adsorção de fosfolipídios e induz a introduzi-los na
monocamada. A SP-C estimula a inserção de fosfolípides e podem
aumentar a resistência do surfactante à inibição por proteínas de soro e
de fluido pulmonar. Em sendo assim, mães tabagistas devem ter a
responsabilidade de fazer uma reflecção dos prejuízos pessoais além dos
prejuízos ao feto incluindo a baixa estatura por hipóxia, por dificuldade
respiratória que causam aos seus fetos, e este fato é perene.

is a key drug in the treatment of infants with a history of respiratory
distress syndrome; it is for this reason important to know the different
types available in the market, its history and properties of, and inform
dosing, drug components and the indications of the types of pulmonary
surfactant that are on the market.
5. Therefore, therapy with exogenous surfactant is widely used in the
treatment of hyaline membrane disease (HMD) in preterm neonates
who require ventilatory support. The development of this therapy has
been an important historical step forward in neonatal intensive care. It
has been studying its beneficial use in other respiratory diseases of the
newborn, such as meconium aspiration syndrome, pulmonary
hemorrhage and bronchopneumonia. The surfactants are four
apoproteins SP-A, SP-B, SP-C and SP-D. SP-A and SP-D are hydrophilic
and SP-B and SP-C are hydrophobic. SP-A and SP-D plays an important
role in the defense against inhaled pathogens and SP-A which also has a
regulatory role in monolayer reduces the surface tension. Hydrophobic
proteins are needed to improve the spread of phospholipids in the air
spaces. SP-B promotes the adsorption of phospholipids and induces
bring them into the monolayer. SP-C stimulates the insertion of
phospholipids and can increase the resistance to inhibition by
surfactants of the serum proteins and pulmonary fluid. That being so,
smoking mothers should have the responsibility to make a reflection of
personal damage beyond damage to the fetus including short stature
due to respiratory distress caused to their fetuses and these facts are
perennial.
