Our projects/opportunities/challenges:
• Medicines form the mainstay of most clinical care and are
critical in chronic diseases.
• Electronic systems are used to manage care in the
community and in hospital but these systems seldom talk
to each other.
• As patients transfer in and out of the hospital system,
there are opportunities for discontinuity - >40% of
medication errors happen at transitions of care.
• Discontinuity causes considerable morbidity and
mortality – patients discontinuing medicines following
hospitalisation have 10% increased 12 month mortality.

• Electronic solutions can improve
continuity.
• Integration of information from various
sources e.g. GP records, nursing homes,
patient held apps.
• Use of patient kiosks and mobile devices
to enter and review medicines
information on admission.
