This survey details key criteria to consider as a lead into self review of any business unit's
performance and operations.
You should complete this survey individually.
Please be honest as all responses are anonymous, and will be collated to provide data for
collaborative discussion.
All the survey's questions are answered on a ratings scale from Strongly Disagree to Strongly
Agree.
There are no text or long answer questions in the survey, these types of comments should be
raised in the collaborative discussion following the survey's completion and principle analysis.
After the survey is closed it is important to allow adequate time for the collaborative
discussion and debate.
The unit will then discuss the Focus on Core Business and two other principles identified as
the greatest opportunities for improvement.
The collaborative discussion will encompass:
a) the evidence used by staff to rate the criteria
b) what the next steps would be for improvement
c) ongoing self review processes
d) the data and other evidence that would be gathered to monitor and evaluate improvement
over time.
