New SAG Mill Risk Assessment
Background
NB Copper mine is one of the biggest copper and gold mine company in Indonesia. Since the
first year of their production, NB produced more than 750.000 ton per annum copper concentrate
and kept growing over time. Furthermore, NB Copper Company targeted to reach 1million ton
copper concentrate in 2019. In order to reach their goal, NB plan to leverage their mill capacity
by add more SAG mill with capacity 750 tph in their processing plant. Previously, they have 2
SAG Mill with capacity 850 tph each.
Context
NB Copper mine company commit to implement good mining practice concept in their
operation, they commit to provide their employee, contractors, guest and everyone working in
their area with safe and convenient environment. Currently, they use OHSAS 18000 as their
safety and environment guidance. Adding new SAG Mill consequently changes their
management particularly in processing unit. Therefore, re-assessment risk is needed in order to
get maximum and effective production target and safety environment.
Objective
This risk assessment has several purposes, those are:
 To assess and identify potential risk in new SAG mining area
 To control potential risk probably occur based on risk identification
 To create safety and convenient environment in mining area particularly processing plant
Scope
This risk assessment focus to identify potential risk which relates to safety and environment
issues. This assessment will identify, evaluate risk probably occur in area of new SAG mill and
formulate control plan.
Risk Assessment Methods:
This risk assessment would like to use 2 risk assessment method, those are:
WRAC, is used because is easiest way to communicate and formulate risk in different aspect
with various people background.
Bow tie, using Bow Tie method is one of the most simple to communicate the risk assessment
with various background people and this method is the easiest way to get helicopter view of risk
description.
In addition, 5 by 5 matrix is used to calculate risk rank.
The Team
 Consultant team to facilitate the assessment risk
 Electrical Engineer to identify potential electrical risk
 Mill Operator to identify potential operation risk
 Superintendent Health, Safety and Environmental Department, to identify potential safety
risk
 Superintendent Processing Plant to identify operation risk in helicopter view
 Production superintendent to identify operation risk relate to crusher operation.
Time and Venue:
In order to get maximum and effective result, the risk assessment project will be held 2 days
from 3-4 may 2017. The first day all of team will visit the area of new SAG mill in processing
plant to get the data of concentrate supply from crusher, condition of the area, SAG Mill
electricity supply and sources, the distance between SAG mill and others equipment and objects.
The second day the team will do a meeting in processing plant workshop room.
Expected Output and Risk assessment feedback
The expected output in this project is a document which contains risk assessment result.
Therefore, the related division can do some action regarding those risks.
This assessment risk will give to all parties to get feedback. Risk assessment is the last process
but iterative process, so feedback is very important to make this risk assessment more
comprehensive.
