#EndPJparalysis 100 Day Challenge - Briefing for Teams
If you had 1000 days left to live, how many would you choose to spend in hospital?
Why?
• Time is the most important currency in healthcare.
• Wasted time manifests, for instance, in patients waiting, duplication, staff running
around looking for things, and needless patient harm being caused as a result of time ill-
spent waiting.
• Among older patients receiving care in our hospitals, 46% of people > 85 years die within
one year of admission to hospital (Clark et al 2014).
• Deconditioning due to prolonged bed stays in hospitalised older patients causes serious harm
and steals patients’ time, especially those who may be in their last 1000 days.
• Across the NHS there are several key initiatives in various degrees of design, implementation
and success leading to sustainable change. These include:
#endPJparalysis, #Red2Green, #Last1000days, #Fit2Sit
The Last 1,000 Days (Dolan 2016) emerged from the construct of patient time as the most
important currency in healthcare.
#EndPJparalysis 100 Day Challenge - Briefing for Teams
What are we doing?
We are setting ourselves a 100 Day Challenge. We plan to create a sustainable movement and
prepare our teams for winter.
 From 14 September 2017 to 22 December 2017: Across East of England region in just 100
days we are aiming for 100,000 patient days up, dressed and moving in their own clothes,
rather than wearing hospitals gowns or PJs.
 In 2018: we aim to be even more ambitious. Go nationwide. Support a million patient days of
#endPJparalysis in hospital settings.
The East of England is in the vanguard of this movement.
Benefits of #endPJparalysis include: (and you may wish to look at these in your Trust)
• Reduced LOS (< 0.75 days in one PDSA at Hillingdon Hospital)
• Reduced loss of mobility, deconditioning and risk of falls
• Reduced food wastage due to greater patient mobility and energy need
• Reduced risk of needing institutional care on discharge
• Reduced use of continence products
• Enhanced wellbeing of patients and staff
• Promotes early discharge
We are working with NHSI analytics to capture further metrics and it will be measurement for
improvement and not judgement

This work is being led by a team including:
• Ciara Moore, COO, Queen Elizabeth Hospital, King’s Lynn
• Professor Brian Dolan, Director, Health Service 360, Honorary Prof of Leadership in
Healthcare, University of Salford and originator of #Last1000days and #EndPJparalysis.
The 100 day challenge builds on work created by Lisa Sharp, Project Manager, NW Anglia NHSFT
and co-led with clinical leaders across the East of England region.

What we need from you
• Collect your data of the number of patients up, dressed and moving from 14 Sept-22 Dec in
your ward/unit level – you will be sent an electronic spread-sheet to capture your data at 12
midday each day of the challenge.

• your local transformation or agreed local Trust teams and
• the NHSI analytics team at nhsi.improvementanalyticsteam@nhs.net
• Share your stories and pictures on the Last 1000 days Facebook page, Twitter, Academy of
NHS Fab Stuff and with us! On Twitter please use the hashtags:
• #EndPJparalysis and
• #PowerOf1000 – to remind people of the importance of the last 1000 days and the
power we collectively have to enable patients to spend more time at ‘home’

  • 2. #EndPJparalysis 100 Day Challenge - Briefing for Teams If you had 1000 days left to live, how many would you choose to spend in hospital? Why? • Time is the most important currency in healthcare. • Wasted time manifests, for instance, in patients waiting, duplication, staff running around looking for things, and needless patient harm being caused as a result of time ill- spent waiting. • Among older patients receiving care in our hospitals, 46% of people > 85 years die within one year of admission to hospital (Clark et al 2014). • Deconditioning due to prolonged bed stays in hospitalised older patients causes serious harm and steals patients’ time, especially those who may be in their last 1000 days. • Across the NHS there are several key initiatives in various degrees of design, implementation and success leading to sustainable change. These include: #endPJparalysis, #Red2Green, #Last1000days, #Fit2Sit The Last 1,000 Days (Dolan 2016) emerged from the construct of patient time as the most important currency in healthcare.
  • 3. #EndPJparalysis 100 Day Challenge - Briefing for Teams What are we doing? We are setting ourselves a 100 Day Challenge. We plan to create a sustainable movement and prepare our teams for winter.  From 14 September 2017 to 22 December 2017: Across East of England region in just 100 days we are aiming for 100,000 patient days up, dressed and moving in their own clothes, rather than wearing hospitals gowns or PJs.  In 2018: we aim to be even more ambitious. Go nationwide. Support a million patient days of #endPJparalysis in hospital settings. The East of England is in the vanguard of this movement. Benefits of #endPJparalysis include: (and you may wish to look at these in your Trust) • Reduced LOS (< 0.75 days in one PDSA at Hillingdon Hospital) • Reduced loss of mobility, deconditioning and risk of falls • Reduced food wastage due to greater patient mobility and energy need • Reduced risk of needing institutional care on discharge • Reduced use of continence products • Enhanced wellbeing of patients and staff • Promotes early discharge We are working with NHSI analytics to capture further metrics and it will be measurement for improvement and not judgement
