Sequentia’s platform offers healthcare providers the tools
and data to optimize transition of care. Our proprietary
algorithms and interfaces minimize rehospitalizations as
well as maximize patient satisfaction and quality of care.
The seamless system was developed in conjunction with
care providers so it is easy to incorporate into existing
workflows, reducing paperwork and workload while
placing patients in the next phase of care setting.
Sequentia delivers a ‘middleware’ solution with a
predictive, data-driven approach to support hospitals’
clinical and operational objectives related to Transition of
Care. In support of interoperability, Sequentia connects
hospital and post-acute care provider EHR systems and
additional software applications. The platform is a cloud-
based, HIPAA compliant tool that collects relevant patient
data from the EHR and analyzes the impact of $8.4 billion
Medicare claims to give the Hospital the ability to:
• Identify the patients that are most likely to be
readmitted - within hours of admission
• Communicate information on these ‘high risk’ patients
to the appropriate clinical and business staff
• Provide details and choice around the optimal post-
acute care provider based on specific patient data and
diagnosis
• Utilize an electronic, bi-directional communication tool
to facilitate the referral process
• Measure and report on all relevant elements of the
Transition process including referral patterns and re-
sponse times, rehospitalization rates, related ICD-10

The future of a bundled payment model in the US
healthcare system is well-documented in the lead up to
and launch of Medicare’s Comprehensive Care for Joint
Replacement Program (CCJR). Following closely is the
almost certain expansion to DRGs beyond 469 and 470.
In conjunction with the bundled model, the cost attribu-
tion for post-acute care, Medicare Spend Per Beneficiary
(MSPB), is also receiving keen focus. The seven areas
of MSPB drive necessary change in the relationships
between acute and post-acute care providers.
Payment
Without a strategy and system to identify ‘high value’
post-acute care providers, both Skilled Nursing and
Home Health (low rehospitalization rates, cost effective,
etc.), particularly in a bundled payment environment,
hospitals are exposed to increased financial risk as reim-
bursement will now be pegged to the national average.
