潘美連 (Mei-Lien Pan) 台灣醫學資訊學會祕書長潘美連2001年畢業於陽明大學公共衛生研究所，取得碩士學位；2013年取得博士學位。目前是台灣醫學資訊學會秘書長，並曾經在2009-2013年擔任台灣健康產業整合協會秘書長，也是該會的創會秘書長，積極參與各項醫學資訊的推廣與研究工作。她同時也是台北護理健康大學兼任助理教授，主要教授醫學資訊概論、健康照護資料處理、資料倉儲等課程。主要研究興趣包含健康照護資料處理與分析、醫學資訊與標準、公共衛生。

何謂大數據

試問：生病看醫生時，醫生若能對照病人的基因體，是否能提供更精準的治療？但每個人都有4百多萬個變異點，到底那個才是跟疾病相關的呢？ [用DNA看世界系列] 總覺得世界上發生一些事件是因為人們的無知或私心而造成的，譬如之前轉貼過的 momondo 的 The DNA Journey [1]，藉由 DNA 的分析化解人種間的歧視或誤解。但有時還是會看到新聞說發生族群或宗教而進行種族淨化 (Ethnic Cleansing) [2,3]，相信將 DNA 攤開，全世界都是一家人，試問，你還會對你的家人做出這麼殘忍的事嗎？前陣子因為蔣宗哲教授的邀約，叫我準備個題目跟高中生分享，我就在想可以用 DNA 來講什麼故事，希望能帶給高中生一些不同的體驗或看法，所以回想一下我高中時代，除了籃球和聯誼外，還有什麼回憶。其實，高中時代晚上在 K 書時都會聽廣播，有一次聽到一個高中生 Call-In 進來說他是同志，來自父母和同學們的壓力，不知道該怎麼辦？那時候的教育與學到的知識，會把同志與 AIDS 畫上等號，所以那時覺得他生病了。現在，我覺得這是不對！尤其上個月發生美國11州告歐巴馬政府[4]，因為頒布了「跨性別廁所使用指南」，要求全國各級學校讓跨性別學生「依自己所認定的性別」來使用公共廁所，而不是依照出生時的「生理性別」。而上週又發生奧蘭多槍擊同志酒吧事件[5]，只因為看到同志在街上親吻。我們的世界不能再發生這類的事了，所以我就想挑戰一下這個議題。我想用 DNA 來解釋同性戀其實跟其它疾病一樣，都是因為 DNA 上一些變異點造成的，所以只要找到有論文提出這類的證據，這樣的功德圓滿了。但很不幸的，可能對這個領域還不熟，沒有找到直接的證據說同性戀是因為那個或那些基因上有變異而造成的。所以我試著用其它證據來說明，第一個要挑戰的是人類就只有異性戀和同性戀兩種嗎？答案當然不是，性學專家金賽博士早在1948年就提出了「金賽量表」(Kinsey Scale)[6]，明確定義出 7 種類型，認為每個人都有一定程度的同性戀／雙性戀傾向，差別只是在於程度上的問題。根據當時統計，20至35歲的白人男性中，有11.6%在這個年齡階段時性取向為「等級3－異性戀與同性戀傾向相同」。所以，從心理層面來分，這是個多類別分類的問題，而非二分法，我甚至覺得這個問題是該做 regression，而非 classification。第二個想要挑戰的是大家用性染色體來區分男性 (XY) 與女性 (XX)，也可以簡單地判斷是否有 Y 染色體來分辦，有 Y 就是男性，沒有 Y 就是女性。但事實上是否這個單純呢？這個就好玩了，找到一篇 2015 年在 Nature 發表的論文 [7]，標題叫「Sex redefined」，內容提到一位 46 歲懷孕的婦女身體內卻有 Y 染色體，進一步再歸納出就染色體來分類，也可以分為 7 個類別，所以大家習慣的傳統性別分類其實大粗糙了。所以，若美國11州要告政府通過跨性別廁所使用問題，那我也可以去控告那11州，他們沒有未其它 5 類的性別準備好廁所給他們使用，因為要分成 7 種類型的廁所才完全不會互相干擾、造成心理上的不適。您說是嗎？雖然這次分享，同學們有點不給面子的睡到流口水，只有在贈送小禮物問答時候才活過來，但我覺得我己經盡我所能地提供不同面向的看法，傳遞給同學生。相信我下次可以講得更好，有興趣的朋友可以參考我的投影片，更歡迎大家給我指教，讓世界變得更美好。 PS: 其實這次分享最怕的是有同學因為聽完我的分享後，就回家跟父母親直接講說他／她是同性戀。若可以，我更希望跟學生家長分享這次演講內容。 [1] http://www.momondo.tw/ins…/dna-journey-celebrate-diversity/… [2] http://www.ettoday.net/news/20140809/387803.htm [3] http://www.appledaily.com.tw/realtimenews/…/20151211/750876/ [4] http://www.appledaily.com.tw/realtimenews/…/20160526/870840/ [5] https://tw.news.yahoo.com/%E5%A5%A7%E8%98%AD%E5%A4%9A%E6%A7… [6] https://zh.wikipedia.org/…/%E9%87%91%E8%B3%BD%E9%87%8F%E8%A… [7] http://www.nature.com/news/sex-redefined-1.16943
