I/WE, being the Owner(s), Principal(s), and/or Authorized Agent(s) of the above-named Contractor and Home Modification Provider of record
pertaining to the above-specified Client and associated PAR, do hereby assert and certify the following:
* THAT ALL WORK pertaining to Home Modification under the above-named PAR has been fully and satisfactorily completed in accordance
with the terms and specifications of my/our original Proposal and/or any approved amendments thereto;
* THAT ALL WORK has complied with local and/or state building codes or, if located within an area in which there is no building authority,
that all Work has complied with the appropriate provisions of the 2003 IRC and the accessibility provisions of the 2003 IBC; and furthermore
that all Work has complied with the Colorado Plumbing Code as adopted by the Colorado Examining Board of Plumbers and the National
Electrical Code as adopted by the Colorado Electrical Board, effective 7/1/2005;
* THAT DURING THE COURSE of Work, I/we have conformed to all general certification standards and procedures set forth in 10 C.C.R. 2505-
10, Section 8.487.11;
* THAT NO LIENS have been filed against the PAR or the Project Address in association with the PAR;
* THAT NO SUITS are pending by reason of Work that has been completed under the authority and/or direction of the PAR;
* AND THAT ALL INFORMATION herein contained is true and complete to the best of my/our knowledge.

CLIENT’S STATEMENT OF COMPLETION
I/WE, being the above-named Client or Authorized Representative thereof, do hereby assert and certify that all Work pertaining to Home
Modification as approved under the above-specified PAR has been completed by the above-named Contractor to my satisfaction and/or to the
fullest extent of the ‘Proposed Modification to Home’ specified within the PAR and/or the ‘Scope of Work’ specified within the Contractor’s
approved Proposal. I/we do furthermore acknowledge and certify that any Work not completed as of this date is over and above the Work that
has been covered and completed under the PAR, and that I/we may or may not choose to retain the Contractor to perform any such remaining
Work at my/our discretion, but that full payment for Work performed under and covered by the PAR should now be rendered to the Contractor
in consideration of its completion of the covered Work.

CONTRACTOR’S CERTIFICATE OF WARRANTY
CONTRACTOR DOES HEREBY WARRANT, as of the above date, all labor, workmanship, and materials used in the course of its performance of
the Work associated with the above-specified PAR to be free of material or installation defect for the period of one (1) year from the date of
this Statement and Certificate. Warranty period is to begin immediately upon signature of both Contractor and Client, though invocation of
warranty shall not be considered valid until payment in full for all Work associated with the PAR has been issued and received by Contractor.
IT BEING DULY CONSIDERED that some materials used (e.g. concrete, tile, grout, etc.) are prone to cracking, spalling, and heaving under
certain physical and environmental conditions, Contractor hereby expressly excludes failure of these materials from its warranty excepting
only under such circumstances as it may be fully demonstrable that the failure of these materials was specifically due to material or
installation defect and that all possibility of environmental stress, normal wear, or other physical cause has been ruled out.
IN ALL VALID CASES that this Warranty is invoked, Contractor will repair or replace, at its sole option and cost, any failure in the labor,
materials, or workmanship of the original Work herein specified.
