A strategy to prevent readmissions and improve care for patients with psychiatric conditions at risk for readmission

3. Major Problem with Psychiatric
Services
The high utilization of services by a
select group of patients residing in
Assisted Living Facilities (ALF’s) who are
readmitted constantly.

4. CST (Crisis Stabilization Team)
 Psychologist
 Psychiatry Resident
 RN
This team will attend to calls from ALF’s and will
make frequent visits to ALF’s.

5. CSU (Crisis Stabilization Unit)
 Stabilize patient and discharge back to ALF
 Establish new ones in areas with most need.
 Work collaboratively with existing CSU’s
Community Mental Health Centers

6. CST/CSU Objectives
 Identify barriers and issues that lead to frequent
hospitalizations and readmissions
 Educate the Assisted Living Facilities’ staff
 Reduce inpatient hospitalization
 Stabilize patients in the least restrictive environment as
possible
 Prevent inpatient hospitalization
 Decrease the expense of treatment
