This document discusses the DSM-5 Parent/Guardian-Rated Level 1 Cross-Cutting Symptom Measure-Child Age 6–17. The measure consists of 25 questions that assess 12 psychiatric domains completed by the parent/guardian to rate their child's symptoms over the past 2 weeks. Nineteen items use a 5-point severity scale, while suicidal ideation, suicide attempts and substance abuse use a yes/no scale. Scores on each item within a domain are reviewed to determine the highest score, which clinicians note to aid further inquiry. The measure is intended to aid clinical decision-making and diagnosis but not be the sole basis for diagnosis.
