team
The Advocacy Project is an independent, rights based
advocacy provider and conforms to the definitions of
an independent provider as described in the Scottish
Independent AdvocacyAlliance (SIAA) Principles and
Standards for Independent Advocacy organisations.
The Engagement and Involvement team (E & I team) of
The Advocacy Project provides an independent service
which support people to be meaningfully involved in
the service they receive.
It promotes an ethos of empowerment, participation
and involvement on behalf of those who use services.
We work across Glasgow, North Lanarkshire, South
Lanarkshire and East Renfrewshire.
3. Why
advocacy
and
anticipatory
care
planning?
Thinking about what you may need relating to care and
support in the future can be daunting for many
reasons.
Being able to have a discussion with someone who is
both impartial and knowledgeable.
We can support people to complete their ACPs or
empower clients to complete them independently with
the correct information.
We can support people to liaise with a range of
professionals if needed to complete their ACP.
4. Glasgow &
South
Lanarkshire
Pilots
Provided information to residents at identified
sheltered housing complexes in Glasgow and South
Lanarkshire
Provided follow up one to one surgeries for residents to
have discussions and input information into their
anticipatory care plans.
Liaised with relevant health professionals, family
members and others with residents if needed in
relation to inputting information in to ACP.
5. Our
ongoing
progress?
Provided information sessions to residents of complexes in the
Lanark, Rutherglen, Blantyre and Carluke areas.
Information sessions arranged in Hamilton and East Kilbride.
Completed surgeries in Lanark and Carluke.
1 to 1 surgeries arranged in Blantyre and Rutherglen.
Hosted information providing sessions on advocacy,ACP and
POA, to 30 residents.
Supported 5 residents in the South Lanarkshire area to fill out ACP
forms. A further 6 felt confident completing forms without
advocacy support.
Sent 11 GP letters to request ACP consultation on behalf of
residents.
