What is Transition Planning?
From a system perspective:
Legal Mandates IDEA 2004 & TEC 29.011-
• Beginning not later than the first IEP to be in effect when
the student turns 14 or younger if determined appropriate
by the IEP team & updated annually thereafter. (TEC)
• The IEP must include appropriate measurable
postsecondary goals based upon age appropriate transition
assessments related to training/education, employment,
and where appropriate independent living skills (IDEA
300.320 (b) (1)).

From a system perspective:
Measuring Post-School Outcomes (SPP 14):
Percent of youth who had IEPs, are no longer
in secondary school and who have been
competitively employed, enrolled in some type
of postsecondary school, or both, within one
year of leaving high school.
2012-2013 SPP Results:
• Out of 13,319 graduated students from
Texas
• 4,308 responses were gathered
• 59% reported not being employed or enrolled
in post secondary education
• 68% reported not contacting any adult
service agencies for support

  1. I want to thank you for being here today. Our plan today is to take each of you on a journey. A journey that will hopefully challenge you and your mindsets. For today I would like to ask you to put your day to day stressors aside and focus upon what we plan to share with each other and where we plan to go through this process. Let’s go over a few of the things we will be utilizing today and make sure you feel prepared and packed for our journey. As we travel we want to make sure we all stay together…so if you have a question don’t hesitate to ask… we will try to answer it for you. If we don’t know, we will try to point you to a resource to get you to the information you need. I am Stephanie… education for over 18 years….I have experience as a general educator (4th grade & 6th grade ELA), special education behavior teacher, special education counselor, Licensed Specialist in School Psychology, and five years experience as a special education administrator. I recently developed PAST, in these initial stages we provide training sessions such as this as well as contracting services for schools in assessments, FIEPs, counseling services and consulting. We are building momentum toward a five year plan to create a skill building center for individuals with disabilities in transition.
  2. As we think about transition planning it’s important to keep in mind each of our participates are going to be coming from at least 1 of 3 perspectives or viewpoints. Over the next 45 minutes we will review the importance of each of these perspectives and the role they play in the planning process.
  3. The importance of a team in PCP is to gather the experts on the specific perspectives and settings…. One of the key elements of Person-Centered Planning is listening and piecing together the perspectives to have understanding…. This video is an illustration of why we need to take the time to gather more information….
  4. Time should about 9:00 It depends on the individual. Activity: (two minute song) Define success to you. What are the qualities of life that make life good to you? Share your thoughts with your table or with the group…. When you think about the “appropriate” goals to enhance the quality of your life how did you know? Through experiences and opportunities you begin to discover and understanding… this is the approach of PCP.

  8. PCP team is part of your relationship & places map. The history map is important in knowing & understanding perspectives to move forward- this discussion is where fears are revealed. Through the relationship map, you create a network to support expanding gifts & interests and creating a vision for the future. Opportunities are key to future success…
