underserved rural and urban communities
•Awards up to $100,000 for education loan
repayment in return for three years of service in an
underserved area
6. • Physicians (MD or DO) providing primary care
(FP, IM, Peds, OB) and Psychiatrists
•Dentists, Hygienists, NPs, PAs, CNMs
•Working full time in a Health Professional
Shortage Area, a federally qualified health
center, tribal health center or rural site for three
years
•Full time: 32 clinic hours/week, 45 weeks/year (21
hours/week for OB)
7. Physicians are eligible for either:
•Up to $50,000 if practicing in a HPSA, or
•Up to an additional $50,000, if practicing in a rural
HPSA, for a total potential award of $100,000, or
•Up to $50,000 if practicing in a rural area but not in a
HPSA.
•Rural: population less than 20,000 and at least 15 miles from
anywhere that has a population of 20,000+
8. • Three annual payments
• Two party check to you and your lender
• All awards are state and federal income tax
exempt and require a three year service
obligation

Wisconsin Department of Health Services
Designation based on:
• Population : Provider Ratio
• Poverty level
• Distance to nearest provider
• Specific need factors (high % elderly, alcohol use, etc.)
