Medicare and Medicaid originally financially drove America’s system of long-
term care. For 40 years, most planning and design for old age in the United
States came from inadvertent financial incentives, which were poorly designed
and have come back to haunt America’s seniors. Many other nations look to the
United States for leadership; however, the way in which America has
approached this rapidly ageing population offers multiple lessons on what not to
