Bank, Kelambakkam for Rs.12349/- (Rupees Twelve Thousand Three
Hundred & Forty Nine Only) towards full and final settlement of all my
claims including all statutory and other payments from M/s TDYAS
ENTERPRISES, Kelambakkam and the Principal Employer viz.,
M/s RAMSAYS CORPORATION PVT LTD, 13/1,A-B, Thiruvalluvar Salai,
Kumaran Nagar, Sholinganallur, Chennai – 600 119,.
I hereby accept the full and final settlement amount to my fullest satisfaction
of all my claims / demands – both statutory and other payments and I assure
that I shall not raise any claim or demand against M/s TDYAS
ENTERPRISES, Kelambakkam or M/s RAMSAYS CORPORATION PVT
LTD, 13/1,A-B, Thiruvalluvar Salai, Kumaran Nagar, Sholinganallur,
Chennai – 600 119,.
