REGISTRATION
• Must be Filipino
• Of legal age
• Good moral character
• NBI clearance
• Medical certificate
• High school graduate
• Certificate of Training

4. VALIDITY OF
DOH MASSAGE LICENSE
• 3 years

5. When to Renew License?
• Birth month on
3rd year

6. Requirements for License Renewal
• Health certificate
• Professional tax receipt
• Cedula
• DOH fee
• Continuing Education (30
units)
• Submission of documents to
DOH regional office

7. Implication for Non-Renewal
• Revocation of license
after 5 years of non-revalidation

8. Steps to Earn 30 Units - FREE
• Organize 10 participants (former
AWSA students)
