Stonebridge Care LLC provides quality senior care in Boise, Idaho. StoneBridge Medical Case
Managers will help you and your family navigate the medical and psycho-social challenges associated
with caring for an older loved one. Our case management services integrates health care and
psychological care with other needed services such as: housing, home care services, nutritional
services, assistance with activities of daily living, socialization programs, as well as financial and legal
planning (e.g. banking, trusts). A tailored care plan is prepared after the individual assessment and is
continuously monitored and modified as needed. Our elder case managers have training in nursing,
social work, gerontology or other health service areas.
We help individuals, families and other caregivers adjust and cope with the challenges of aging or
disability. We can assist with care-planning assessments, in-home help, counseling and support,
referrals to specialists, and moving clients to or from a retirement complex, assisted living facility,
