Use “5 Whys” to probe beyond superficial causes to identify root

5 WHYS
• 5 Whys is a question-asking technique used to explore the
  cause-and-effect relationships underlying a particular problem
• The primary goal of the technique is to determine the root cause of
  a defect or problem
• Developed by Toyota to address manufacturing issues in Lean
  Manufacturing

• TIPS:
    - There can be several causes to a problem
    - Don’t stop before you reach root causes
    - Use hypotheses for your best guess, to be validated later

Use “5 Whys” to probe beyond superficial causes to identify root causes Root Cause 5 WHYS • 5 Whys is a question-asking technique used to explore the cause-and-effect relationships underlying a particular problem • The primary goal of the technique is to determine the root cause of a defect or problem • Developed by Toyota to address manufacturing issues in Lean Manufacturing • TIPS: - There can be several causes to a problem - Don’t stop before you reach root causes - Use hypotheses for your best guess, to be validated later © 2007-2012 IES Development Ltd.
