These slides are from a presentation at the Think Local Act Personal conference on 26th November 2014. The slides and short film provide an introduction to the Coalition for Collaborative Care and its vision for a better deal for people with long-term conditions.
3. Our vision for the future
There is an increasing recognition of the great
potential for new and better ways for people’s
expertise to come together to ensure improved
outcomes for people with long-term conditions.
But also frustration that the promising practice that
can be seen in patches around the country is not
becoming mainstream practice quickly enough.
We want to change this and work with those
determined to move person-centred collaborative
care from the margins, to the centre of practice and
experience.
4. How do we plan to do this?
We want to create the right
conditions for this to flourish,
through:
• Workforce development
• Providing powerful evidence
• Identifying and pulling system levers to
drive positive change in practice
5. How do we plan to do this?
It is about supporting and nurturing a powerful
movement for change amongst practitioners,
people living with long-term conditions, their
family carers and the wider community.
Ensuring this ‘movement’ has the tools,
connections and support it needs.
And that people with long-term conditions and
their family carers are at the heart of this.
6. How do we plan to do this?
• We will play an important part in key developments that will shape the future of health
such as the Integrated Personal Commissioning Programme
• We will look for all opportunities to get significant support targeted to those local
systems showing clear commitment to transform practice. This includes support to the
people with long-term conditions so they are able to contribute to this movement
• We will support our members to become better informed, to link together and develop
their ability to have positive local impact and we will consult with members on how
best to do this
7. Creating sustainable impact
There is an opportunity to make a real
difference to the lives of people with long-term
conditions.
But for deep, large-scale, sustainable change
to happen we need to creative a collective,
powerful movement.
We already have a strong group of national
partner organisations and we’ll be actively
encouraging additional partners and members
to join.

8. TLAP and C4CC
• The role TLAP has played, initially in social care has inspired the development of C4CC
• TLAP has broadened its role into health reflecting the need for personalisation to happen
across public services
• C4CC’s roots are in health – work done in recent years to develop different and more
useful relationships between health professionals and people with long term conditions
and to show how people themselves can take greater charge of their health in their
communities. C4CC sees an opportunity to bring organisations and people from the health
world into much greater engagement with social care
9. Working together
Accordingly we have started to explore some
of the areas where synergy makes real sense
and spot joint projects and initiatives flowing
from this. At this stage we are planning
together in several areas:
• The Integrated Personal Commissioning
programme
• Person centred care and support planning
• Health and Well Being Board Framework
• Implementation of the integration
elements of the Care Act
In order to maximise impact at regional and
local level we will also be exploring
opportunities to make mutual use of regional
events, networks and connections
We will be exploring areas of mutual interest
in influencing policy and policy delivery
For both alliances co-production is core to
design and delivery of our work and we will
aim to connect up here too.
