3. At 10am on Sunday, Chattopadhyay, a
production engineer, entered the machine
with Goyal and Singh to clean it while
Senapati was at its entry point. Just then,
someone in the factory's control room
switched on the machine. By the time
Senapati jumped off and managed to turn the
switch off, Singh had started spinning with
the machine and died on the spot.
4. Two staffers of Reshmi Metalis factory in
Kharagpur died when a machine was switched on
while three people and a production engineer
were busy cleaning it on Sunday. When the
machine restarted, three people were inside
cleaning it. Rajkumar Singh, 28, was on the
machine's mixture he it started spinning. He
spun with it and died instantly. The factory's
production engineer Monojit Chattopadhyay, 28,
and worker SP Goyal, 22, too were inside while
Karunamoyee Senapati, 30, was about to enter.
Chattopadhyay succumbed to his injuries in a
Kolkata hospital on Monday.
5. Operations at the factory were suspended
since Monday.
The company's AGM Surendra Jha said the
victim's kin would be compensated in
accordance with rules. He, however, claimed
that the factory was operating.
7. A 22 year old, Ambiga had opened the inspection
door of the loading machine when it got stuck.
She was supposed to call the floor leader for
such work but opened it herself. If the door is
opened, the machine switches off automatically.
But it can also be operated even if the doors are
open and the girl had switched it on, unmindful
of the danger. Her neck got trapped between the
door and the tray. When she screamed, the
others switched off the machine and broke open
the trays to rescue her.

respect, we closed our facility in Chennai on
November 1st and members of our senior
management have met with the family of our
colleague.
We are investigating this tragic incident thoroughly
and we are also collaborating with local authorities in
India with full transparency. While it would
inappropriate and insensitive to the family of our
colleague to comment on the details of this accident
until the full facts are known, we can say that the
details being reported by some online media and
tabloid publications are simply untrue.”

11. The Bhopal disaster, also referred to as
the Bhopal gas tragedy, was a gas
leak incident in India, considered the world's
worst industrial disaster. It occurred on the
night of 2–3 December 1984 at the Union
Carbide India Limited (UCIL) pesticide plant
in Bhopal, Madhya Pradesh. Over 500,000
people were exposed to methyl
isocyanate gas and other chemicals. The toxic
substance made its way in and around
the shanty towns located near the plant.
