2. Solution Overview
 High Level Solution Description:
• Beacon was developed to help medical practices better manage and
care for patients tied to accountable care organizations (ACO).
 Healthcare/Business problem addressed:
• There is a great need in the marketplace for an integrated software
solution that helps providers at the point of care manage ACO patient
populations. Beacon not only tracks quality processes but outcomes,
provides patient risk scoring and tiering and breaks out total and
expected costs.
 The application developed addresses the following
category:
 Applications that improve management of high cost chronic diseases.
 Innovative approaches to addressing value-based care imperatives.

3. Usability and Design
 Beacon will be integrated into AllScripts and the staff upon rooming a
patient will immediately be able to see if the patient has issues that need
to be addressed. When the provider meets with the patient he/she can
review problem areas and make recommendations. Providers can create
lists on the fly of high and moderate risk patients and actions can be
taken to bring the patient back in for a follow-up.
 We want providers and staff to take a more proactive approach to patient
care. This can be done by having the right information at the point of
care.

5. Integration Description
 We will place an icon in the context bar next to the portal
icon. The icon will change based on the risk/health status of
the patient in context
 We want to make it as easy as possible to address problems
and metrics that are in alert status right from the application.
We will use: SaveTask, SaveProblem, SaveResult, SaveRx,
SaveVitals, and SaveImmunization

 The video highlights the problem doctors face with the
growing amount of information they must siphon through
as more patient data becomes available to them. The
introduction of Accountable Care Organizations only
exacerbates the problem. The video then demonstrates how
‘beacon’ addresses that problem and helps the doctor find
the data most relevant to them and meet their ACO goals.

8. Why this solution be selected!
 Most providers and clinicians have no idea where a patient goes when
they leave the office. With the advent of ACO programs, providers now
have this data, but don’t know what to do with it. We feed this data
back to providers along with quality information through an integrated
solution with the AllScripts eMR.
 We feel more and more patients will be tied to ACO’s and systems will
need to be put in place to manage these patients. We feel we have the
best solution to help clinicians maintain a higher level of care while
reducing costs.
