Elements of neonatal life
       support
          
 drying and covering the newborn baby to conserve
  heat;
 assessing the need for any intervention;
   
 opening the airway;
     
 aerating the lung;
       
 rescue breathing;
         
 chest compression;
           
 administration of drugs (rarely).
             
Guide line changes
               
 For uncompromised babies, a delay in cord clamping of at
  least one minute is now recommended.
 For term infants, air should be used for resuscitation at
  birth, oxygen should be used when needed guided if
  possible by pulse oximetry.
 blended oxygen and air should be given judiciously and
  its use guided by pulse oximetry
 Preterm babies of less than 28 weeks gestation should be
  completely, covered up to their necks in a food-grade
  plastic wrap or bag, without drying immediately after birth
Guide line changes 2010
           
 compression: ventilation ratio for CPR remains at 3:1
  for newborn resuscitation
 aspirate meconium from the nose and mouth of the
  unborn baby, while the head is still on the perineum,
  are not recommended
 If apneic baby is born through meconium, inspect
  the oropharynx, then endotracheal intubation and
  suction.

                    
 If adrenaline is given then the intravenous route is
  recommended using a dose of 10-30 mcg/kg if given
  intratrachealy 5-10 times that dose is needed.

 moderate to severe hypoxic – ischaemic
  encephalopathy should, where possible, be treated
  with therapeutic hypothermia.
Life support
                   
 Before the baby is born check your equipment, and
 take history.
 Clamp the umbilical cord.
 Put baby under radiate warmer, dry the baby,
 remove damp cloth, stimulate the baby.
 put baby in neutral position, suction through the
 mouth then the nose, if needed.

   This all that most babies need, if the baby is
breathing well, pink, H.R > 100
Advanced support
             
 If gasping or not breathing: Open the airway, Give
  5 inflation breaths Consider SpO monitoring
                            2


 If chest not moving: Recheck head position,
  Consider other maneuver.
 If heart rate is not detectable or slow (< 60 min-
  1),Start chest compressions, 3 compressions to each

  breath
 Reassess heart rate every 30 s, If heart rate is not
  detectable, or slow (<60 min-1) consider venous
  access and drugs
Airway;

place the baby on his back with the head in the neutral position, i.e.
with the neck neither flexed nor extended. Most babies have a prominent
occiput causing flexion of the neck, to minimize this we can put a folded
cloth under the shoulders, but avoid over extension.




 Breathing;


 • If the baby is not breathing adequately give 5 inflation breaths, preferably
    using air.
 • If the heart rate does not increase, and the chest does not passively move
 with each inflation breath, then you have not aerated the lungs
If aeration of lung not effective
                          
 If the lungs have not been aerated then consider:
 Is the baby’s head in the neutral position?
    
 Do you need jaw thrust?
      
 Do you need a longer inflation time?
        
 Do you need a second person’s help with the airway?
          
 Is there an obstruction in the oropharynx (laryngoscope
            
  and suction)?
 What about an oropharyngeal (Guedel) airway?
              
 Is there a tracheal obstruction
                
Chest compression;


 In babies, the most efficient method of delivering chest compression is to grip
 the chest in both hands in such a way that the two thumbs can press on the
 lower third of the sternum, just below an imaginary line joining the nipples,
 with the fingers over the spine at the back.
 Compress the chest quickly and firmly, reducing the antero-posterior diameter
 of the chest by about one third. The ratio of compressions to inflations in
 newborn resuscitation is 3:1.




Drugs;


 Drugs are needed rarely and only if there is no significant cardiac output
 despite effective lung inflation and chest compression.
Drugs
                     
 adrenaline; 10-30 mcg/kg intravenously or 30-50
 mcglkg intratrachealy.

 sodium bicarbonate; 1-2 mmol/kg (2-4 ml of 4.2% sol)

 Dextrose; 2.5 ml/ kg 0f 10% glucose water

 volume replacement; 10ml/kg 0.9% normal saline in
  10-20 sec.

Life support   Before the baby is born check your equipment, and take history.  Clamp the umbilical cord.  Put baby under radiate warmer, dry the baby, remove damp cloth, stimulate the baby.  put baby in neutral position, suction through the mouth then the nose, if needed. This all that most babies need, if the baby is breathing well, pink, H.R > 100

Advanced support   If gasping or not breathing: Open the airway, Give 5 inflation breaths Consider SpO monitoring 2  If chest not moving: Recheck head position, Consider other maneuver.  If heart rate is not detectable or slow (< 60 min- 1),Start chest compressions, 3 compressions to each breath  Reassess heart rate every 30 s, If heart rate is not detectable, or slow (<60 min-1) consider venous access and drugs
  • 8. Airway; place the baby on his back with the head in the neutral position, i.e. with the neck neither flexed nor extended. Most babies have a prominent occiput causing flexion of the neck, to minimize this we can put a folded cloth under the shoulders, but avoid over extension. Breathing; • If the baby is not breathing adequately give 5 inflation breaths, preferably using air. • If the heart rate does not increase, and the chest does not passively move with each inflation breath, then you have not aerated the lungs

If aeration of lung not effective   If the lungs have not been aerated then consider:  Is the baby’s head in the neutral position?   Do you need jaw thrust?   Do you need a longer inflation time?   Do you need a second person’s help with the airway?   Is there an obstruction in the oropharynx (laryngoscope  and suction)?  What about an oropharyngeal (Guedel) airway?   Is there a tracheal obstruction 

Chest compression; In babies, the most efficient method of delivering chest compression is to grip the chest in both hands in such a way that the two thumbs can press on the lower third of the sternum, just below an imaginary line joining the nipples, with the fingers over the spine at the back. Compress the chest quickly and firmly, reducing the antero-posterior diameter of the chest by about one third. The ratio of compressions to inflations in newborn resuscitation is 3:1. Drugs; Drugs are needed rarely and only if there is no significant cardiac output despite effective lung inflation and chest compression.

  • 14. Drugs   adrenaline; 10-30 mcg/kg intravenously or 30-50 mcglkg intratrachealy.  sodium bicarbonate; 1-2 mmol/kg (2-4 ml of 4.2% sol)  Dextrose; 2.5 ml/ kg 0f 10% glucose water  volume replacement; 10ml/kg 0.9% normal saline in 10-20 sec.
