 Health organization or location level designees can enter orders and
view results for single providers or groups of providers

STEP 1: SET UP YOUR ONLINE PORTAL ACCESS

Easy access to your account:
 Complete and SIGN* this form, even if you are currently
faxing your orders
 Assign providers and designees for your health
organization ‐ eachemail address will receive an invite
 Set up a username/password to access your account
 Health organization designees may administer orders
and results based on selected permission(s) for portal
access
 Designees may use their permissions to administer for
EACH provider listed for the health organization
 View your complete order history on the first login
*The signature acknowledges responsibility that access is limited to providers and designees for use permitted by HIPAA guidelines.
