Classification of pulmonary hypertension
ในปี พ.ศ. 2551 การแบ่งชนิดของ PH โดยพิจารณาจาก clinical evolution และ
histopathology (5)
และการตอบสนองต่อการรักษาโดยจัดให้ systemic left to right shunt
อยู่ใน Class I: PAH โดยเพิ่มคำจำกัดความของ Class II: โรคของหัวใจห้องซ้าย (left

Management in Pulmonary Hypertension
แนวทางการรักษาผู้ป่วยที่มีภาวะ PAH ที่ปรับปรุงใหม่ โดยใช้ ACCP (American
College of Chest Physician) grading system ประกอบด้วยการใช้ quality of evidence
(good, fair, low หรือ expert opinion) ร่วมกับ net benefit ว่าเป็น large, intermediate,
small, non, conflicting หรือ negative) ตามตารางที่ 4 โดย grading ของ
recommendation แบ่งเป็น level A ดีที่สุด จนถึง D หรือ I (intermediate) และแยกส่วน
expert opinion ออกไป แผนภูมิต่อไปนี้แสดงการรักษาที่แนะนำสำหรับปี พ. ศ. 2552 ให้
พิจารณาการสวนหัวใจห้องหวา (right heart catheterization: RHC) ในผู้ป่วย iPAH เนื่อง
จากอาจมี response ต่อ calcium channel blocker (CCB) และสามารถใช้ยานี้ได้ในระยะ
ยาว สำหรับผู้ป่วย PH จากสาเหตุอื่นให้ดูตามความจำเป็นของแพทย์ผู้รักษา โดยใช้แนวทาง
การตอบสนองต่อ acute pulmonary vasodilator testing ร่วมกับการจำแนกว่าผู้ป่วยมีอัตรา
พยากรณ์โรคที่ดีหรือไม่ ตามชนิด low หรือ high risk (ตารางที่ 4)
