CRIMINAL HISTORY

FCYFS, PLLC. requires pre-employment criminal history background checks. This action is taken to ensure that the organization is not negligent in hiring individuals
who may pose a threat to the persons served or staff of the organization.
Applicants that are offered employment will be informed that said employment will be contingent upon obtaining a criminal history/background check from the
Oklahoma State Bureau of Investigation (OSBI) or other approved organization prior to beginning employment. The applicant/employee/contractor is responsible
for the cost of the criminal history/background check. The Clinical Director may decide to rescind the offer of employment based on the report findings. No
clinical staff involved in service delivery will be made an offer of employment or contract should background check findings have violations involving
offenses to children or domestic violence, or on a case-by-case basis that may create a reason for concern.
All criminal history background reports shall be treated as confidential information. If an applicant provides any inaccurate information, that person will not be
considered further for the position. If the organization learns that any information about a criminal history provided by a current employee/contractor is
inaccurate, that employee/contractor is subject to discipline. However, employee/contractors will be given an opportunity to explain any inaccuracies. If a
satisfactory explanation is not given, the employee/contractor may be immediately terminated from employment.

CRIMINAL HISTORY FCYFS, PLLC. requires pre-employment criminal history background checks. This action is taken to ensure that the organization is not negligent in hiring individuals who may pose a threat to the persons served or staff of the organization. Applicants that are offered employment will be informed that said employment will be contingent upon obtaining a criminal history/background check from the Oklahoma State Bureau of Investigation (OSBI) or other approved organization prior to beginning employment. The applicant/employee/contractor is responsible for the cost of the criminal history/background check. The Clinical Director may decide to rescind the offer of employment based on the report findings. No clinical staff involved in service delivery will be made an offer of employment or contract should background check findings have violations involving offenses to children or domestic violence, or on a case-by-case basis that may create a reason for concern. All criminal history background reports shall be treated as confidential information. If an applicant provides any inaccurate information, that person will not be considered further for the position. If the organization learns that any information about a criminal history provided by a current employee/contractor is inaccurate, that employee/contractor is subject to discipline. However, employee/contractors will be given an opportunity to explain any inaccuracies. If a satisfactory explanation is not given, the employee/contractor may be immediately terminated from employment.
