Problem: Risk for Diabetes
After completing a Health Risk Assessment, Ann realized she was past due for
a physical exam and promptly scheduled an appointment. Her doctor told her
she had high cholesterol, and that she was pre-diabetic. She was also told she
needed to lose weight, change her diet and exercise.
Solution: Ann’s Health Advocate Wellness Coach suggested
ways to eat healthier and use her lunch break to walk. In
addition, a Personal Health Advocate helped her locate a
diabetes education class.
