MNO MiPCT Meeting

   Meeting held on December 12, 2012 had 
    representation from all MiPCT practices
   Presentation of six month metrics: process
   Roll out of year one metrics: clinical outcomes

Six Months Metrics

   Assess hiring and training of care managers
    • Majority of practices rely on MNO care managers
    • Two practices employ RNs; however none have 
      attended care manager training
   Assess status of patient registry
    • In place
    • Training in the use of the patient registry completed

   Assess care manager (moderate, complex, hybrid)

Practice Assessments

   Each practice will be visited by clinical directors 
    and operations director
   Lori Zeman, PhD to visit each MiPCT to assess 
    behavioral health integration

   Certified coder workshops for physicians and 
    teams
   Appropriate use of ICD‐9 Codes
   Advanced Care Planning
   Durable power of attorney

Care Management Reporting

   The care management activity reporting 
    requirements have been finalized and have been 
    incorporated into the Quarterly Report beginning 
    in the first quarter of 2013
   Track number of encounters

Patient Advisory Council

   A Patient Advisory Council (PAC) is being convened 
    as an advising resource to the Steering Committee, 
    subcommittees and other MiPCT groups 
   The goal is to ensure that the patient voice is 
    incorporated in the implementation and operations 
    of the MiPCT

Patient Advisory Council

   The council will be comprised of patients serviced by 
    the MiPCT
   MiPCT is seeking nominations for a state‐wide PAC 
    which is expected to meet approximately three to four 
    times per year
   In‐person attendance is preferred, though phone 
    participation can be accommodated
   Members will be eligible for mileage reimbursement

Criteria for Selection

   Members should be
    • MiPCT patients (especially patients who have 
      experience with care managers)
    • Able to use their own experience constructively
    • Able to see beyond their own experience
    • Able to listen to and hear differing opinions
   Member nominations can be submitted at:

   The nomination period ends on February 28

    management into the workflow of MiPCT
    practices.

  • 3. MNO MiPCT Meeting  Meeting held on December 12, 2012 had  representation from all MiPCT practices  Presentation of six month metrics: process  Roll out of year one metrics: clinical outcomes 3

Six Months Metrics  Assess hiring and training of care managers • Majority of practices rely on MNO care managers • Two practices employ RNs; however none have  attended care manager training  Assess status of patient registry • In place • Training in the use of the patient registry completed 4

  • 6. Practice Assessments  Each practice will be visited by clinical directors  and operations director  Lori Zeman, PhD to visit each MiPCT to assess  behavioral health integration 6

  • 8. Care Management Reporting  The care management activity reporting  requirements have been finalized and have been  incorporated into the Quarterly Report beginning  in the first quarter of 2013  Track number of encounters  8
  • 9. Patient Advisory Council  A Patient Advisory Council (PAC) is being convened  as an advising resource to the Steering Committee,  subcommittees and other MiPCT groups   The goal is to ensure that the patient voice is  incorporated in the implementation and operations  of the MiPCT 9

Patient Advisory Council  The council will be comprised of patients serviced by  the MiPCT  MiPCT is seeking nominations for a state‐wide PAC  which is expected to meet approximately three to four  times per year  In‐person attendance is preferred, though phone  participation can be accommodated  Members will be eligible for mileage reimbursement 10

Criteria for Selection  Members should be • MiPCT patients (especially patients who have  experience with care managers) • Able to use their own experience constructively • Able to see beyond their own experience • Able to listen to and hear differing opinions  Member nominations can be submitted at:  https://jodyooo.wufoo.com/forms/patient‐advisory‐council‐ nomination‐form/  The nomination period ends on February 28 11
