This document outlines a reentry program called "Change the Home, Change the Cycle" that aims to empower women leaving incarceration. It provides mentoring and referral services for 365 days after release. Navigators will help the women reconnect with families and children through one-on-one and group mentoring. The program will train mentors and match them with participants leaving a women's correctional facility. It establishes a process for intake, support services, and interviews to evaluate outcomes. The goal is to serve 33% of participants in the women's reentry program and implement this navigator program starting in January 2010.
