Exit Plans Fostering Connections to Success and Increasing Adoptions Act of 2008 Title II, Section 202 Mandates that 90 days prior to emancipation from foster care, “a caseworker on the staff of the State agency, and, as appropriate, other representatives of the child provide the child with assistance and support in developing a transition plan that is personalized at the direction of the child, includes specific options on housing, health insurance, education, local opportunities for mentors and continuing support services, and work force supports and employment services, and is as detailed as the child may elect.”

Dear Lisa, I just wanted to reiterate how grateful we at JLC/YFC are for your support of Youth Fostering Change and the youth-developed discharge hearing form. We really appreciate your passion for easing the transition to adulthood for youth in care. Thank you for spreading the word about YFC’s project through Foster Club. We’re also thrilled that you want to encourage the Ohio courts to implement the form. If we can provide any assistance with this, please let us know. We’ll have a new group of YFC members in the fall and expect to continue to engage our past members to be advocates for system change. It would be wonderful to find a way to work with youth from Ohio on a project. Sadly, Yowei and I will both soon be leaving JLC.
