COA by the numbers - 2010 20,145 clients receiving in-home services  Nearly 2 million home-delivered meals (to 11,371 homebound older adults) 1.9 million hours of in-home care 253, 167 trips to doctors, senior centers 21,580 requests for information about help for seniors 321 education and support sessions with caregivers 46 advocacy meetings with clients and elected officials From 2010 annual report
II. Long-term care:  what is it and why does it matter?
What is long-term care?
What islong-term care? It’s help with basic activities of daily living for an extended period of time. Activities of Daily Living (ADL)  Instrumental Activities of Daily Living (IADL)
In the past… Long-term care  took place in a  nursing home.
Today… People age 65+:  Only 5% live in group quarters, like nursing homes Source: AARP/U.S. Census Bureau Credit: Alyson Hurt/NPR
Settings for long-term care In own home or caregiver’s home Group homes or informal congregate arrangements Subsidized housing for seniors Adult day center Continuing Care Retirement Community Assisted Living – stand-alone or part of CCRC or nursing home Nursing home
Why the change? Better health and longevity Medical advances Universal design and assistive technologies  Long-term care insurance  Caregiving/cultural shift Quality of life expectations, desire for independence and privacy Continuing-care retirement communities

Even if you think you won’t Only 37% of seniors think they will need LTC. In fact, about 70% actually will need care for some period of time.  90+% of seniors have not discussed w/ spouse or children how they will pay for care, or what care options they prefer.
Long-term care matters Reason #2: if you’re a taxpayer, you’re already paying for long-term care.

III. How can Council on Aging help you?
COA Programs and Services “I love being able to stay in my own home.  It means something to my heart.” - Ruth
Typical home care client Single woman in her 70s or 80s Needs help with bathing, cooking, cleaning, dressing, shopping Lives on a fixed income Family and friends pitch in to help out, but it’s not enough Family is concerned it’s not safe for her to live at home by herself
COA home care: PASSPORT and Elderly Services Program Care management Adult day services Caregiver respite Companion service Emergency response device Homemaking Home-delivered meals Home modifications Independent living assistance  Personal care Major housecleaning/pest control Medical equipment and supplies  Transportation
PASSPORT Prevents unnecessary nursing home placement Must be at least 60 years old Low income and assets (Medicaid-eligible) Frail : Need hands-on help with dressing, bathing, toileting, grooming, eating or mobility (2 or more of these)  Able to remain safely at home with doctor’s consent  Ineligible for services from another source: Medicare, private pay, insurance or hospice Free of charge or set at affordable level depending on financial situation Served 3,851 in 2010 (5 counties)
PASSPORT client “Without the program, I’d be in a nursing home somewhere. I cannot do for myself…but I do have people who love me.”    	Katie, PASSPORT client with her COA care manager
Elderly Services Program (ESP) County-based in-home care for those who do not qualify for PASSPORT  Allows wider range of disabilities than PASSPORT Income is not an eligibility factor Available in Butler, Clinton, Hamilton and Warren counties (Clermont- separate program) Supported by local tax levies – you pay for this service, you should use it! Served 14,283 in 2010 (4 counties)
ESP continued… 65 or older (60 in Warren Co.) Impaired in everyday activities such as bathing and preparing food Free of charge or set at affordable level depending on income Flexible, individual care plan + care management Caregiver support Ineligible for services through another source/payer
Elderly Services Program client “I thought I could do without help but I found out I couldn’t.  I would tell people, ‘Don’t wait as long as I did to get help.’ I don’t know what I did without Pat (home health aide).” Nellie and Noble, Hamilton County ESP clients, with aide Pat
Assisted Living Waiver Private, apartment living with supportive services. Provides care for Medicaid-eligible adults in approved assisted living facilities.  An alternative for people who need more care than PASSPORT or Elderly Services Program, but not a nursing home. Marian and daughter Carol “I love it here. Assisted Living Waiver Services: Private rooms w/bath and kitchen 24-hr on-site response Meals Care Management Homemaking Personal Care Social Activities Community Transition Service
Assisted Living Waiver Eligibility: age 21 or older Need same level of care as PASSPORT (frail) And currently: A nursing facility resident, or receiving care through another Medicaid waiver program (PASSPORT), or an assisted living resident who has paid privately for at least six months
Home-delivered Meals Based on nutritional, not financial need Age 60 or older Unable to prepare a meal or get to a congregate meal site Spouses eligible Donations welcome

Help for caregivers Council on Aging Caregiver Support Program Homevisit from RN for teaching about medications, diabetic care, bathing, stress and more Must provide care for someone 60+ OR be a caregiver over the age of 60 Instruction manual and resource guide All offered as a public service
HOME: Repairs/Modifications Free home repairs and safety modifications for low-income homeowners and renters age 60+ Funded by Ohio HousingTrust Fund Money available!

IV. Fair Care Ohio: Getting involved
Advocacy
Focusing on the issues We work with elected officials and leaders on issues of importance to older adults and caregivers.  Balancing Ohio’s long-term care system Promoting choice Saving tax dollars Improving quality of life for older adults Housing Bed bugs
Advocacy Victory Rebalancing Long-term Care in Ohio Fair Care Ohio: Nearly 50,000 Ohioans spoke up so older adults could have choices for long-term care Ohio’s 2012-13 budget increases PASSPORT and Assisted Living spending by $55.6 million over the next 2 yrs and allows 4,800 more seniors to access these programs Ohio’s long-term care spending balance will soon be 58% to 42% (institutional vs. home/community care)
Still more to be done… Ohio still exceeds the national average in nursing home admissions Ohio’s 60+ population will grow 25% by 2020 and will nearly double by 2040  The new budget puts us closer to the national average, but we can do better Ohio’s Area Agencies on Aging want a 50/50 balance of spending on nursing homes and home/community-based care = more care options for Ohioans and $$ savings for taxpayers

Still more to be done… Ohio still exceeds the national average in nursing home admissions Ohio’s 60+ population will grow 25% by 2020 and will nearly double by 2040 The new budget puts us closer to the national average, but we can do better Ohio’s Area Agencies on Aging want a 50/50 balance of spending on nursing homes and home/community-based care = more care options for Ohioans and $$ savings for taxpayers
