2. The Case
A 23-years old male patient was
admitted to Sulaymaniyah Teaching
Hospital, Sulaymaniyah, Region of
Kurdistan, Iraq on 28th of March 2009
because of productive cough of 1
month duration. CXR (PA view)
revealed a cystic lesion with (water-
lily sign) in the superior segment of
the left lower pulmonary lobe.

7. The Case…
The patient was operated upon via
left posterolateral thoracotomy.
The contents of the cyst (hydatid
fluid and laminated membrane)
were removed. The bronchiolar
fistulae were individually closed by
2-0 silk sutures. The cavity was left
open after sealing all fistulae. The
patient had a smooth postoperative
recovery. 7
11. Comment
● The water-lily sign, also known as the
camalote sign, is seen in hydatid
infections when there is detachment of
the endocyst membrane which results in
floating membranes within the pericyst
that mimic the appearance of a water lily.
This sign is pathognomonic of intra-
bronchial rupture of PHC.
