Medical
Dr Peter Wilmshurst (Royal Shrewsbury Hospital, UK), refused to
sign off on the MIST paper, in part because he was never given full
access to the trial data. He also protested the fact that the Circulation
paper made no mention of a second echo review that the trial sponsor
had, in fact, asked him to conduct.
Wilmshurst's echo review, never published, pointed to a higher residual shunt
rate than the 6% reported in the MIST manuscript.
Late last year, the British High Court ordered NMT to pay £200 000 in security
to the court to cover the legal costs for Wilmshurst should NMT lose its libel suit
against him, an amount ultimately raised to £211 000.

19. Tutte le volte che una MDD compare al termine di immersioni “non a rischio”, cioè
eseguite rispettando le procedure decompressive, e la sintomatologia compaia
entro 30’ dal termine della immersione si deve sospettare la possibilità
di un PFO.
Come indicazione generale, il PFO deve essere considerato come una
controindicazione assoluta alla attività subacquea, solo quando lo stesso diventi
sintomatico al termine di una immersione considerata “non a rischio”.

Nessuno ha cercato di identificare quali sono le caratteristiche
che aumentano la probabilità che il PFO sia etiologicamente
correlato all’ictus.
L’inclusione di una quota significativa di pazienti in cui il PFO
rappresenta un reperto solo incidentale,ha probabilmente
impedito di evidenziare il beneficio conferito dalla terapia
endovascolare sul trattamento medico.

Si deve a Botallo medico italiano che lavoro' nelle corti europee , la scoperta di una comunicazione intracardiaca , anche se rimase oscura la ragione di cio'. Fino ad oggi e' rimasto il termine di forame di Botallo.
A Julius Cohnheim spetterebbe il primato di avere ipotizzato, nel 1877, che la mancata chiusura del forame ovale dopo la nascita , sarebbe stata la via tramite la quale gli emboli, potevano raggiungere la circolazione cerebrale
Ma solo negli anni ottanta con la tecnologia dell'ecocardiogafia e' stato proposto che il PFO possa giocare un ruolo importante
Dal punto di vista anatomico e ontogenetico il forame ovale non è un vero proprio buco, ma ha le caratteristiche di un tunnel, risultando dalla mancata fusione di due foglietti, il septum primus ed il septum secondum
Ormai nella terminologia corrente, quindi non solo medica, lo shunt destro-sinistro , cioè il passaggio di sangue non ossigenato dal comparto venoso a quello arterioso , viene associato al PFO e per comodità procederò in tal senso . Ma è necessario tenere a mente che uno shunt destro-sinistro può essere causato anche da una comunicazione fra arteria e vena polmonare, per esempio.
