Carol Rasmussen, MSN, NP-C, CDE is a nurse practitioner with many years of experience treating patients with diabetes. Currently Ms. Rasmussen practices at the Exodus Healthcare Network in Magna, Utah and also serves on the AADE Editorial Advisory Board for The Diabetes Educator publication. Moreover, Ms. Rasmussen received the Legislative Leadership Award from the American Association of Diabetes Educators at their 2009 Conference in Atlanta.
Her presentation will cover the challenges of increasing access to diabetes education and strategies for overcoming such obstacles, as well as various tools/resources/programs from AADE.Adherence

It is important for patients to carefully consider their lifestyle, daily routines, support systems and any barriers to determine the best regimen and adherence strategies. Carol Rasmussen, MSN, NP-C, CDE is a nurse practitioner with many years of experience treating patients with diabetes. Currently Ms. Rasmussen practices at the Exodus Healthcare Network in Magna, Utah and also serves on the AADE Editorial Advisory Board for The Diabetes Educator publication. Moreover, Ms. Rasmussen received the Legislative Leadership Award from the American Association of Diabetes Educators at their 2009 Conference in Atlanta.
Her presentation will cover the challenges of increasing access to diabetes education and strategies for overcoming such obstacles, as well as various tools/resources/programs from AADE.Adherence
Taking HIV medication requires strict adherence to a complex regimen of multiple daily pills and dietary restrictions. Staying adherent is challenging and many factors can interfere. It is important for patients to carefully consider their lifestyle, daily routines, support systems and any barriers to determine the best regimen and adherence strategies. This may involve trialing strategies like using pillboxes or alarms, integrating medications into daily habits, and planning for disruptions to routines. Building a strong relationship with medical providers and understanding the treatment plan is also key to maintaining adherence.
Medication Reconciliation
Learn best practices based on literature and how to perform a complex and accurate medication history. Recognize gaps/inconsistencies in systems that impede medication reconciliation and identify next steps in improving current medication reconciliation within your own practice.

Preparing for a diabetes checkup helps patients get the most from their limited time with doctors. Patients should bring:
1) A list of all current medications including dosage and times taken.
2) Recent blood sugar readings from their meter to show how treatment is working.
3) A written list of questions about concerns or issues since the last visit.
Preparing ensures doctors have the needed information to properly assess treatment and make any necessary changes to improve patient care and health outcomes.

- Only about 1/3 of patients follow their doctor's treatment plans exactly as prescribed. The lack of follow through frustrates doctors.
- Healthcare professionals believe that with proper motivation and coaching between visits, most patients would take steps to improve their health. However, there are often too large of gaps between patient appointments.
- A possible solution is for doctors to use technology like automated reminders and messaging to provide ongoing encouragement and support to patients between visits in order to help patients better follow treatment plans and make progress toward their health goals.

Suggestions to address these challenges included setting reminders, taking pill organizers everywhere, keeping pictures and information about each medication, and using apps or journals to track diet and exercise modifications.
