The best way to think about Azure RTOS is not just as a real-time operating system, but as a comprehensive suite for developing embedded IoT applications. It is a set of industry-leading multithreading facilities and middleware that enable embedded developers to quickly build real-time software—that is, software for constrained devices requiring small footprint, preemptive, deterministic, or time-bounded capabilities with predictable response times.
At its core is Azure RTOS ThreadX, a small, fast, reliable real-time operating system (RTOS) that is already powering more than 6.2 billion devices worldwide. It also includes these other Azure RTOS components, all of which are fully supported and easy-to-use—which is perfect to help developers get to market faster.
All Azure RTOS components are built as loosely coupled C libraries, so that only what the application actually uses is brought into the final image at link time. This virtually eliminates the need for complicated configuration and build parameters, making things easier for the developer.
Azure RTOS can easily pull in the extra security from Azure Security Center with seamless connectivity. That means you can:
Manage the same security posture across all on-premises and cloud workloads
Prevent, detect, and remediate attacks on all IoT deployment components
Continuously monitor the security of IoT devices, machines, networks, and Azure services
Automatically discover and onboard new devices, and apply security policies to ensure compliance with security standards
Azure RTOS also connects fully with Azure IOT Central. That means you can:- Get a fully-managed IoT application development platform
Take advantage of built-in disaster recovery, multitenancy, global availability, and predictable cost structure
Easily connect and manage your devices
Generate insights and bring those insights into your line of business sapplications
Azure RTOS provides truly industrial-grade connectivity to the cloud with its components and supported protocols. It uses the near-wire-speed performance and small footprint of NetX Duo, along with the IoT cloud protocol Message Queue Telemetry Transport (MQTT) to publish and subscribe data to the cloud. All data in both directions is secured by NetX Secure TLS. Any physical layer transport layer is supported, e.g., Ethernet, WiFi, 802.15.4 radio, and more.
Azure RTOS also provides more protocosl for industrial networking than any other commercial RTOS

This is a snapshot comparison between the three Microsoft OS offerings: Azure RTOS, Azure Sphere, and Windows 10 IoT. To further understand the difference between Azure Sphere and RTOS, consider the following:Azure Sphere provides everything an embedded developer needs to build a highly secured device - devices that are secure-by-default and fully prepared for the evolving cybersecurity challenges of the internet. While Azure Sphere chips include real-time processing cores that are protected by the Azure Sphere OS, Azure Sphere does not provide the RTOS for embedded developers to take advantage of these real-time processing cores. This is where Azure RTOS comes in.
