• Het botverlies is multifactorieel: gewicht, eiwit tekort,
vitamine deficienties, hormonale assen,
darmhormonen
• Enkelvoudige therapie bijv.oestrogeen of
bisfosfonaat, heeft matig resultaat
• Ghrelin en PYY zijn goede voorspellers van BMD

• BMD T-score -3.1
• Gezond, eet normaal, beweegt normaal
• Gewicht 54 kg bij een lengte van 179 cm, BMI 16,8

• Er is een duidelijk leeftijdeffect:
risico op heupfractuur
< 75 > 75 jaar
Camp survivors 9,4 % 7,9 %
Non-camp survivors 2,2 9,5
Controlepersonen 0,8 7,3
Het effect is vooral zichtbaar bij <75 jaar dwz degenen
die aan het einde van de oorlog ,≤ 17 jaar waren.

36. Conclusie
• De effecten van ondervoeding op het skelet hangen
sterk af van de context: leeftijd, duur, hormonale
veranderingen.
• Wanneer ondervoeding optreedt tijdens de groei is er
geen volledig herstel en kan het fractuurrisico nog
tientallen jaren verhoogd zijn.
• De ontstaanswijze van osteoporose bij anorexia
nervosa is complex; naast voedingsdeficientie en
geringere belasting spelen veel hormonale
mechanismen een rol.
• Bij anorexia nervosa kan naast optimale voeding en
oestrogeen een bisfosfonaat overwogen worden.
