Scottish School of Primary Care Evaluation of
Primary Care Transformation
• The Primary Care Transformation Fund (PCTF)
invested £20 million on testing new models of
primary care (part of a £60 million fund covering
additional aspects of care such as mental health,
community pharmacy, and out-of-hours care)
starting April 2016 for 2 years
• The Scottish School of primary Care (SSPC) is
helping to evaluate these new models of primary
care (2017 – 2018)
National and Local Levels
• The national level evaluation will include the Scottish
Governments own theories of change and expectations of
impact and own indicators of impact – ongoing work
• Detailed evidence of Impact, learning, spread and
sustainability will be mainly gathered through a limited
number of selected local case studies (‘deep dives’) carried
out by SSPC members in different Health Boards or specific
themes
• Complemented with the available data and evidence from
the other sites not selected for detailed case study. In this
way, an integrated and detailed sharing of learning will be
produced which will be of national as well as local
relevance.

Phase 1: Intervention Theory and
Expectations of Impact:
The key questions include:
• What are the planned interventions/projects/service
developments and how do they build on previous work?
• What are the key components of the intervention/project?
• Are these likely to change over the life of the intervention?
• What are the expected impacts in the short, medium, and
long-term?
• How do the stakeholders think these impacts are going to
be achieved?
• What is the evidence to support this?
• Who are the key stakeholders in terms of future
sustainability and spread and what evaluation information
do they require?
Phase 2: Impacts, Learning, Spread
and Sustainability
The key questions include:
• What impact(s) has the intervention/project/programme
had, in relation to the expected impacts to date?
• Has the intervention, and the expected impacts changed
over time?
• Have there been any unintended negative consequences?
• What is the key learning that needs to be shared?
• Which interventions seem worth scaling up and spreading?
• How easily can these be implemented?
• How sustainable are these likely to be in the long-term?
Evaluation of New Models of Primary Care
Inverclyde Case Study January 2018
Inverclyde was selected in September 2015 to
pilot new models of care, including GP
Clusters, ahead of the release of funding to
other Scottish NHS Health Board areas in April
2016.
The evaluation by SSPC was carried out between 1
October 2016 and 31 March 2017.

Findings
• The functioning of the GP Clusters (12-18 months later)
remains at an early stage. Their intrinsic work in quality
improvement is likely to emerge relatively quickly, but
their extrinsic role, especially in engagement with the
Integrated Joint Boards, may take much longer.
• The 6 tests of change conducted to date have generally
involved a few GP Practices within or across GP
Clusters. There has been limited engagement of the
wider primary healthcare team or the public in the
choice and design of the tests of change. The potential
effects of new models of primary care on health
inequalities is unclear.
Key Learning
• Healthcare staff require substantial support for data
collection, extraction and analysis
• The internal role of GP Clusters in relation to improving the
quality of care for their combined patient populations
requires time to develop new trusting relationships.
• Their external role in relation to reorienting the NHS in
Scotland towards integrated new models of primary care
requires close collaborative working with IJBs.
• Early and comprehensive engagement with the wider
healthcare team and with service users is of paramount
importance in moving forward. It is also important to ensure
that the most deprived communities benefit as much as the
more affluent.

necessary but not sufficient…….
Delivering the Evidence-Base Primary Care
Transformation
• There is a compelling ongoing need to fill the
many ‘evidence-gaps’ in integrated primary care.
• There is an important innovative ‘middle-ground’
that sits between the current remit of national
research funding bodies and service evaluations.
• A focus on this ‘research middle-ground’ – working
closely with the NHS and social care partners –
could provide evidence within a relatively short
time frame to inform primary care transformation
and to help realise Realistic Medicine.

Scottish School of Primary Care Evaluation of Primary Care Transformation • The Primary Care Transformation Fund (PCTF) invested £20 million on testing new models of primary care (part of a £60 million fund covering additional aspects of care such as mental health, community pharmacy, and out-of-hours care) starting April 2016 for 2 years • The Scottish School of primary Care (SSPC) is helping to evaluate these new models of primary care (2017 – 2018)

National and Local Levels • The national level evaluation will include the Scottish Governments own theories of change and expectations of impact and own indicators of impact – ongoing work • Detailed evidence of Impact, learning, spread and sustainability will be mainly gathered through a limited number of selected local case studies (‘deep dives’) carried out by SSPC members in different Health Boards or specific themes • Complemented with the available data and evidence from the other sites not selected for detailed case study. In this way, an integrated and detailed sharing of learning will be produced which will be of national as well as local relevance.

Phase 1: Intervention Theory and Expectations of Impact: The key questions include: • What are the planned interventions/projects/service developments and how do they build on previous work? • What are the key components of the intervention/project? • Are these likely to change over the life of the intervention? • What are the expected impacts in the short, medium, and long-term? • How do the stakeholders think these impacts are going to be achieved? • What is the evidence to support this? • Who are the key stakeholders in terms of future sustainability and spread and what evaluation information do they require?

Phase 2: Impacts, Learning, Spread and Sustainability The key questions include: • What impact(s) has the intervention/project/programme had, in relation to the expected impacts to date? • Has the intervention, and the expected impacts changed over time? • Have there been any unintended negative consequences? • What is the key learning that needs to be shared? • Which interventions seem worth scaling up and spreading? • How easily can these be implemented? • How sustainable are these likely to be in the long-term?
  • 13. Evaluation of New Models of Primary Care Inverclyde Case Study January 2018
  • 14. Inverclyde was selected in September 2015 to pilot new models of care, including GP Clusters, ahead of the release of funding to other Scottish NHS Health Board areas in April 2016. The evaluation by SSPC was carried out between 1 October 2016 and 31 March 2017. http://www.sspc.ac.uk/media/media_573766_en.pdf
  • 15. Findings • The functioning of the GP Clusters (12-18 months later) remains at an early stage. Their intrinsic work in quality improvement is likely to emerge relatively quickly, but their extrinsic role, especially in engagement with the Integrated Joint Boards, may take much longer. • The 6 tests of change conducted to date have generally involved a few GP Practices within or across GP Clusters. There has been limited engagement of the wider primary healthcare team or the public in the choice and design of the tests of change. The potential effects of new models of primary care on health inequalities is unclear.
  • 16. Key Learning • Healthcare staff require substantial support for data collection, extraction and analysis • The internal role of GP Clusters in relation to improving the quality of care for their combined patient populations requires time to develop new trusting relationships. • Their external role in relation to reorienting the NHS in Scotland towards integrated new models of primary care requires close collaborative working with IJBs. • Early and comprehensive engagement with the wider healthcare team and with service users is of paramount importance in moving forward. It is also important to ensure that the most deprived communities benefit as much as the more affluent.

Delivering the Evidence-Base Primary Care Transformation • There is a compelling ongoing need to fill the many ‘evidence-gaps’ in integrated primary care. • There is an important innovative ‘middle-ground’ that sits between the current remit of national research funding bodies and service evaluations. • A focus on this ‘research middle-ground’ – working closely with the NHS and social care partners – could provide evidence within a relatively short time frame to inform primary care transformation and to help realise Realistic Medicine.
