2. • PP : หญิงไทย อายุ 59 ปี
• CC : ล้มมือขวายันพื้นมา 2 วัน
• PI : 2วันก่อน ลื่นล้ม ข้อมือขวากระแทกพื้น มีปวด บวม ข้อมือขวาผิดรูป จึงไปรพช.
ได้ใส่เฝือกที่มือขวา หลังจากนั้นยังมีปวดที่ข้อมือขวาอยู่
• Past illness : U/D – HT
: no food/drug allergy

3. PHYSICAL EXAMINATION
• ABCD - pass
• Vital signs : T36.6 PR 68 bpm BP 185/94 mmHg RR 20
• GA : A middle-age Thai woman, alert, good
consciousness, not pale
• MSK : Rt. Wrist – no wound,
mild swelling, marked
tenderness, limited ROM,
can extend thumb, can do
good, OK, byebye signs,
intact PPS, cap refill< 2sec

4. INVESTIGATION
• Film Rt. Wrist AP, Lateral

5. DIAGNOSIS
•Closed fracture Rt.distal radius
(colles’s fracture)

6. DISTAL END RADIUS
FRACTURE(DER)

7. • Most common orthopedic injury
• Most common mechanism : Falling On an Outstretched
Hand(FOOSH)
• Bimodal distribution
• younger patients - high energy
• older patients - low energy / falls
• 50% intra-articular
• Associated injuries : DRUJ, radial styloid fx (higher energy), soft tissue
injuries in (TFCC , scapholunate ligament, lunotriquetral ligament)
• Osteoporosis
• high incidence of distal radius fractures in women >50
• distal radius fractures are a predictor of subsequent fractures
• DEXA scan is recommended in woman with a distal radius fracture

• Type I,III - most are stable
• Type II,IV,V-unstable

23. ADVICE
• ให้ระดับมืออยู่เหนือหัวใจ เวลานอนควรวางมือบนหมอนหรือวางบนหน้าอก
• ให้ขยับข้ออื่นๆที่ไม่ได้immobilize

24. COMPLICATIONS
• Median nerve neuropathy (CTS)
• Ulnar nerve neuropathy (in DRUJ injuries)
• EPL rupture
• Radiocarpal arthrosis (in articular stepoff > 1-2 mm)
• Malunion and Nonunion
• ECU or EDM entrapment (in DRUJ injury)
• Compartment syndrome
• RSD/CRPS (vitamin C supplementation is
reccommended in postop)
